Description
Sources Sought - The James A. Haley Veterans Hospital, Tampa, FL is issuing this Sources Sought Notice to determine if there are interested and qualified firms to perform a Formulary Analysis and On-Site Risk Assessment. Responses to this sources sought will be used by the Government to make an appropriate procurement decision. The contract would require the Contractor to conduct a comprehensive analysis of the James A. Haley Veterans? Hospital in-patient formulary. The contractor will provide a report indicating pharmaceutical waste stream management recommendations in accordance with federal, state and local hazardous waste regulations. The Contractor shall conduct an on-site risk assessment of the James A. Haley VA Hospital current pharmaceutical waste management practices and policies to develop a gap analysis and determine the level of risk relative to the Environmental Protection Agency?s Resource Conservation and Recovery Act (RCRA). The Contractor will provide a formulary hazard analysis and on-site risk assessment for the James A. Haley Veterans? Hospital, 13000 Bruce B. Downs Blvd., Tampa, FL 33612. Work Requirements - A. Technical: Contractor shall provide all supervision and labor to prepare a hazard analysis of the in-patient formulary and on-site risk assessment for the James A. Haley VA Hospital. Development of the formulary hazard analysis will be based on the Resource Conservation and Recovery Act and all other applicable federal, state and local regulations. Significant coordination between the Contractor and the VA will be required in developing the formulary analysis and onsite risk assessment. B. Inpatient Formulary Analysis: The Formulary Analysis shall include the following information: National Drug Code, Product, Waste Classification, Waste Stream, Waste Code, Additional waste management recommendations. The On-site risk assessment will include the following: An opening conference to discuss the assessment with key managers. A walk-through of the following areas to determine current pharmaceutical waste management practices: Pharmacy, Intensive Care Units, Emergency Department, Operating Rooms, Oncology, Nursing Wards including the Nursing Home, Spinal Cord Injury Unit, Environmental Management Section and Safety. A review of hazardous waste generator status, current waste disposal vendor and pharmaceutical reverse distribution vendors. A written action plan to contain the following information: Action plan based upon the findings with recommendations to include best management practices. Regulatory and interpretive reference to support Contractor recommendations. Additional vendor recommendations. C. Deliverables: (1) 45 days after contract award provide In-Patient Formulary Analysis and written action plan based on the On-Site Risk Assessment to the Green Environmental Management System Coordinator. (2) Brief hospital management and the Environmental Program Coordinator on the Plan. Contact the Contracting Officer Technical Representative (COTR) to arrange date, time and location. Contractor Competency Requirements for the Contractor Personnel developing the Formulary Analysis and On-site Risk Assessment: Work to be completed by a Registered Pharmacist with expertise in federal, state and local hazardous waste regulatory compliance.
