Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. Solicitation VA-797-07-RP-0031, is being issued as a Request for Proposal (RFP). This is a sole-source procurement with First Databank, in accordance with FAR 6.302-1 resulting in a firm-fixed priced, definite quantity type contract. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-15. The NAICS code is 511210 and the small business size standard is 750 employees. The Department of Veterans Affairs has a requirement for a comprehensive database of patient medical information (PMI) sheets and patient warning labels to include: medical condition for which the medication is used; potential drug interactions; proper use of medication (e.g., "take with food"); possible side effects with distinction between those that may, or may not, require medical attention; and proper storage recommendations. A multidisciplinary editorial staff must develop the PMI database. The PMI database must meet requirements of the Omnibus Budget Reconciliation Act (OBRA) of 1990. The PMI database must include both the generic and trade names associated with the drug. The PMI Sheets must be available in both English and Spanish formats, including abbreviated text in both languages. The contractor must supply a file that contains NDC and description of the PMI/Warning Label. This file will be used for matching purposes. The warning label information must be sequenced in order of importance. The provision at 52.212-1, Instructions to Offerors - Commercial Items applies to this procurement. Proposals must include a completed copy of the provision at 52.213-3, Offerors Representations and Certifications - Commercial Items. The clause at 52.212-4 Contract Terms and Conditions applies to this procurement. 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items applies to this procurement. In paragraph (b) of 52.212-5, the following apply: 52.203-6, 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.222-39, 52.225-5, 52.225-13, and 52.232.34. Offer will be evaluated and awarded to the offeror with the lowest price, technically acceptable offer. The contract period will be for one year plus a one year option.
