Description
The VA Montana Healthcare System, Fort Harrison, Montana has a requirement for a contractor or contractors to furnish all management, supervision, labor, equipment, supplies and transportation necessary to perform Laboratory Services which may include Routine (routine, unable to obtain specimen, difficult sticks, etc.) and STAT laboratory tests/specimen processor services (with or without phlebotomy services) for the VA Montana CBOCs located in Anaconda, Bozeman, Great Falls, Kalispell, Missoula, Billings, Miles City, and Sidney, Montana. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-12.     The Request for Proposal is VA-259-07-RP-0048. The North American Industry Classification System Code (NAICS) is 621511 and the small business size standard is considered $10.0M. The Contractor(s) shall provide any laboratory tests properly authorized and requested by a VA-authorized physician assigned to the VA Community Based Outpatient Clinics located in Anaconda, Bozeman, Great Falls, Kalispell, Missoula, Billings, Miles City, and Sidney, Montana. The Contractor(s) shall provide services within the city limits of each CBOC location. The Contractor(s) shall be approved to provide clinical diagnostic laboratory tests under both Medicare and Medicaid regulations and all applicable federal and state laws. a) The Contractor(s) will provide clinical laboratory services to the CBOC and/or Nursing Home during the weekday (7:30 a.m. Mon - 4:30 p.m. Fri) and for Miles City Nursing Home patients only, emergency services when the samples are provided during "off-duty" (4:30 p.m. - 7:30 a.m. Mon-Thurs) and weekends (4:00 pm. Fri - 7:30 Mon). Services to include urgent and routine testing, as requested. A test menu of orderable items is attached, for each test that the contractor lab wishes to offer to the VA from the list, the following information should be provided; Ordering Code (Contractor's test code), CPT Code, Specimen collection and handling requirements, Test offered as STAT (yes/no), Testing site (if not performed at the contractor's site), Unit price and discount offered the government, Assurance that results will be transmitted as soon as released and the reports returned to the VA site as required, such as faxed to multiple sites if needed, Contractor must comply with direct and immediate phone notification of all results meeting VA Montana critical value limits, a list of those limits has been provided. It is acceptable for the contractor to omit from the supplied list any unusual or esoteric test items that will not be available from their lab. In all cases, core laboratory tests used for critical life support decisions must be offered by the contractor. b) At the Bozeman, Great Falls and Sidney sites, contractor will also provide services, when requested, to collect samples from patients (phlebotomy services), process samples as directed using VA-provided instructions and requirements and prepare those samples for shipping to the VA lab(s) for testing. That service shall be provided at a cost-per-patient rate, included in the contractor's bid. When performing this service, all collection and processing supplies for this special service will be provided by the VA. When performing this service in Sidney, the contractor will also be responsible for shipping costs of the samples to the designated VA lab using Fed Ex expedited services and shipping daily for next day delivery. Personnel assigned by the Contractor(s) to perform the services covered by this contract shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia. All licenses held by the personnel working on this contract shall be full and unrestricted licenses. The contract clinical laboratory shall be fully accredited by the College of American Pathologists (CAP), Department of Health & Human Services HCFA Clinical Laboratory Improvement Amendment (CLIA), and/or the Joint Commission on the Accreditation of Hospital Organizations (JCAHO) to be eligible for award. The Contracting Officer contemplates award of one or more Indefinite Quantity (IQ) firm fixed price contracts pursuant to this solicitation. It is anticipated award will be made to multiple contractors. The contract period shall be one year from the date of award with 4 one-year option periods. This procurement is a full and open competition. All interested persons may identify their interest and capability to respond to the requirements. Solicitation documents should be available on or about February 12, 2007. The offeror is responsible for downloading the solicitation and amendments without further notice from the VA, and for monitoring the website for up-to-date information.
