Description

This is a combined synopsis/solicitation issued for commercial items. The VA Network 22 Logistics Office, Long Beach, California 90822 intends to negotiate a commercial (FAR Part 12) sole source contract with Orthopedic Systems, Inc. (OSI), 30031 Adhere Avenue, Union City, CA 94587 under the authority of FAR 6.302-1. A justification for other than full and open competition has been prepared in support of this acquisition. The Request for Proposal #VA 600-07-RQ-0034 entitled "Jackson Orthopedic and Imaging Table System, with components is hereby issued for Commercial items and is prepared in accordance with the format Subpart12.6 as supplemented by additional information included in this notice and incorporates provisions and clauses in effect through FAC 2005-14. The Standard Industrial Classification Code is 5047 and the North American Industry Classification System Code is 423450. VA Loma Linda Healthcare System, has a requirement for an OSI Jackson Spinal Surgical Table System JTS with the following components: ITEM 0001: 1 SYSTEM to include: 1 each 5803 Advanced Control Retractable I base for the OSI Modular Table System; 1 each :ITEM 0002: 5927 Radiolucent Imaging top w/gimbals 500 lb capacity; 1 each :ITEM 0003: 5943AP Jackson Spinal Top (500 lbs) w/gimbals and advanced control pad system; 1 each ;ITEM 0004: 5855 Orthopedic Trauma top 500 LBS PT Weight Capacity w/arc Cart; 1 each; Item 0005: 5356 Pivoting Arm Board 2 inch w/Shear Guard Gel Pad; 1 each Item 0006: 5857 Cross Arm Support; 1 each; Item 0007: 5393 Clark Arm Support; 1 each; Item 0008: 5859 Orthopedic Equipment Cart; 1 each ;Item 0009: 5858 Orthopedic Patient Isolation Drapes 12 /CS; 1 each; Item 0010: 5808 10 Pkit, Jackson Spinal Table 6/CS; 1 each; Item 0011: 5888 Retractor Adapter for Jackson Spinal Table Top; 2 each; Item 0012: 5979-1 Cervical Management Base unit; 1 each; Item 0013: 5808 Jackson Spinal Table Pkit (6/case ); 2 each. This purchase shall also include FOB destination delivery provisions and a one-year warranty, all at no charge. The equipment and component shall be delivered to the VA Loma Linda Healthcare System, 11201 Benton Ave., Loma Linda, California 92357. Offeror shall submit a complete copy of provision of FAR 52.212-3, Offeror Representation and Certification-Commercial Items (Nov 2006). The following Federal Acquisition Regulation (FAR) Clauses and Provisions also apply to the acquisition: 52.212-1, Instruction to Offerors-Commercial Items (Sept 2006); 52.212-4, Contract Terms and Conditions-Commercial Items (Sept 2005); 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders--Commercial Items (Nov 2006). In paragraph (b) of 52.212-5 the following apply: 52.203-6, 52.219-8, 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.222-37, 52.225-1, 52.225-3, 52.225-13; 52.225-15, and 52.232-34. In addition, incorporated herein by reference are the following: VA Acquisition Regulations (VAAR) Clauses and Provisions: 852.236-75, Guaranty (April 1984); 852.211-70, Requirements for Operating and Maintenance Manuals (Nov. 1984). Original proposal shall be submitted on company letterhead along with descriptive literature for all items offered to Brian Van Orman, Contracting Officer (NBC/MP). A determination by the Government not to compete this proposed contract based on responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement. Interested firms will be considered only if they are capable of meeting or exceeding the requirement stated.
