This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. DESCRIPTION OF SERVICE 1.1. SCOPE OF WORK This Performance Work Statement (PWS) describes the work to be performed at the Contractor?s Occupational Health Clinic and/or the 78th Medical Group Occupational Health Clinic. Work site visits at Robins Air Force Base industrial/administrative work sites may be necessary to assess patient?s work environment. The Contractor shall provide a licensed professional team to perform outpatient evaluation and prevention health care services required for Government beneficiaries. The Contractor shall furnish all labor, management, supervision, teaching, consultations and reports, except as provided in this Performance Work Statement. Performance shall be according to the requirements contained in this PWS and professional standards of the Joint Commission on Accreditation of Healthcare Organization or the Accreditation Association for Ambulatory Healthcare. 1.2. REQUIREMENTS Contractor healthcare provider shall perform the following tasks: 1.2.1. Services - Provide outpatient functional job analysis for patients as scheduled by the Government. Patient assessments include both evaluation in the clinic and work site (or recreated work site) to arrive at a placement recommendation. Team composition will be determined by the contractor but must be individualized to patient need. Disciplines represented may include nursing, physical therapy, vocational rehabilitation, ergonomics and/or occupational therapy. The team must be directed by a licensed physician (MD or DO) in the field of physiatry, orthopedics, or occupational medicine. 1.2.2. Medical Documentation - Prepare all documentation to meet or exceed established standards of the Medical Treatment Facility (MTF) in accordance with AFI 44-119, para 2.5.2.4.1 to include: timeliness, legibility, accuracy, content and signature. Medical Records must be complete and accurate and mailed to the Medical Records Sections within 72 hours of patient appointment. The records should be mailed to: 78 Medical Group, ATTN: Functional Job Analysis, Occupational Medicine Services, 78 AMDS/SGPFO, 655 Seventh Street, Robins AFB, GA 31098. Only MTF and Air Force approved abbreviations can be used in documentation of care in the health care record. 1.2.3 Patient Care - Performs complex physical evaluations, tests, measurements, and procedures. Conduct a Functional Analysis with an emphasis on each patient?s physical impairment or limitation. A review is conducted on each patient?s medical history, vocational history, occupation type, Standard Form 78 (Certificate of Medical Examination), and AF Form 1003 (Air Force Standard Core Personnel Document). Measures strength and range of motion of major muscle groups and injured body parts in order to arrive at recommendation(s) concerning an injured employee residual physical function for the purpose of assigning that individual modified work. Plans or develops patient therapeutic programs, if appropriate, and interviews patients to recommend treatment schedules (but refers treatment to the employee?s private physician). Visits the patient?s work place and observes work tasks and working condition. Makes recommendations for adapting work tasks and the work station or placement to accommodate the individual patient?s limitations. 1.2.4 Transferable Skills Analysis ? In the event the Contractor?s evaluation determines that a patient is unable to return to his or her usual occupation, even with reasonable accommodation, the Contractor will perform a vocational transferable skills analysis. The Government will provide a list of alternative jobs for which the patient is otherwise eligible. The contractor will review those jobs and recommend which, if any, of those jobs the patient is physically capable of performing. 1.2.5 Administrative Functions. Monitors and evaluates physical evaluation activities to determine compliance with established standards of patient care, policies, and regulations. Analyzes the requirements for supplies, equipment, and other resources, and makes recommendations. Performs follow up formally on a monthly basis (for up to, but not to exceed, six months) of those employees that were placed in modified work positions to help insure that placements are successful. Attends management directed meetings, such as Placement Review Board (up to two times per month) to make recommendations regarding employee placement. Gathers summary statistics on the number of evaluations that were done and the number of successful placements in modified work. Performs summary six months review of the effort to put light duty employees in modified work positions and writes summary reports to line management, coordinated through the Occupational Medicine Service. 1.2.5.1 Medically Oriented Report ? This report should contain diagnosis, treatment recommendations and details of functional job analysis. Personal Medical Information (as defined by the Health Insurance Portability and Accountability Act of 1996) MAY be used in this report. 1.2.5.2 Personnel Report ? This report should contain detailed recommendations for physical limits with and/or without accommodations, precise details for all accommodations made and, if applicable, the employee?s vocational transferable skills analysis. Personal Medical Information should not be included in this report. 1.2.6 Responsible for creating, maintaining, and disposing of only those Government required records, which are specifically cited in PWS. If requested by the Government, the Contractor shall provide the original record or a reproducible copy of any such records within five working days of receipt of the request. 1.2.7 Patient lists, no matter how developed, shall be treated as privileged information. Lists and/or names of patients shall not be disclosed to or revealed in anyway for any use outside the MTF without prior written permission by the Chief of the Medical Staff. 1.2.8 The providers shall only release medical information obtained during the course of this contract to other MTF staff involved in the care and treatment of that individual patient, or upon the patient?s request to his or her physician or other healthcare provider. The Contractor shall release medical information only in accordance with HIPPA and medical group directives. 1.2.9 The Contractor shall ensure Contractor healthcare providers maintain open and professional communication with members of the MTF. Contractor providers shall respect and maintain the basic rights of patients, demonstrating concern for personal dignity and human relationships. Complaints validated by the QAP and Chief of the Medical Staff, shall be reported in writing to the Contracting Officer and the Contractor for action. Failure of the Contractor to correct validated complaints identified by the MTF staff and the QAP will be considered a failure to perform. 2.0 SECURITY AND PRIVACY REQUIREMENTS 2.1 Flightline Driver?s Training/Vehicle Operations. Contractors, Sub-Contractors, Vendors, commercial delivery companies, and all other private business vehicles will comply with the following requirements while operating a vehicle within the Robins Air Force Base airfield. 2.1.1 MOTOR VEHICLES: Personnel engaged in vehicle operations on the airfield shall be trained and certified in flightline procedures IAW Robins Air Force Base Instruction 13-206. Sponsoring units are responsible for the flightline and industrial area driving training of contractor personnel assigned to or performing work associated with their organization. Certification shall be on AF Form 483 and must be in the possession of the vehicle operator when operating on the flightline. The contractor is responsible for furnishing a trained and certified vehicle operator/escort for all infrequent vehicle operations on the airfield such as material deliveries. The certified escort can be a passenger or operate a lead vehicle no further than three (3) vehicle lengths in front of the escorted vehicle. The contractor shall ensure that all subcontractors comply with these requirements. 3.0 SERVICE DELIVERY SUMMARY Performance Objective PWS Para Performance Standard Perform Transferrable Skill Analysis 1.2.2 Provide report within 72 hours of evaluation.
