Description
This is a request for SF 330?s. THIS IS NOT A REQUEST FOR PROPOSALS. The VA Medical Center, Tomah, Wisconsin, is seeking professional A/E services to prepare schematic design, design development, construction documents, and provide construction period services for Project No. 676-313, Renovate 2nd Floor B-403 for 26 Psychiatry Beds. Building 403 is centrally located on the west side of the VA campus. It is a 3-story reinforced concrete masonry building that was originally constructed in 1947, and was last renovated in 1989. The 2nd floor of Building 403 has approximately 16,470 GSF. The NAICS code is 541330. Estimated value is less than $400,000. This is UNRESTRICTED. The design shall conform to VA design criteria, and shall be based on the necessary site visits, surveys, verification of as-built drawings, schematics (if required), accurate cost time estimates, construction period services, and submittal reviews at completion of the preliminary design (SD), one at completion of design development (DD) and one when the construction documents (CD?s) are completed. Design shall be completed no later than July 1, 2008. Updated as-builts will be required on Mylar and AutoCad 2004 drawing format on high-density diskettes. Standard design criteria established for the Veterans Affairs Facilities to be followed are available at www.va.gov/facmgt/ae. Firms within a 200 mile radius of Tomah, Wisconsin are invited to submit four (4) copies of the SF 330 to the above named Contracting Officer no later than April 2, 2007. Address to submit SF 330 package is: William S. Middleton VA Hospital, Attn: Lydia A. Falk, Contracting Officer, 2500 Overlook Terrace (A-1), Madison, WI 53705-2254. Firms will be considered based on location, technical expertise, and other requirements listed under the evaluation factors. Facsimile copies are not acceptable; however, if available electronically they may be submitted to Lydia.falk@va.gov. If submitted electronically, the Firm is responsible for ensuring delivery. Evaluation factors for consideration of qualifications listed in descending order of importance are: (1) Past performance on similar medical care projects, especially the design of psychiatric in-patient facilities; (2) Overall design experience and capability with architectural, mechanical, and electrical renovation projects; (3) Qualification of staff to be assigned, with special emphasis on field personnel to be utilized; (4) Knowledge in design and construction requirements; (5) Cost control effectiveness, maintaining project schedules and project budgets; (6) Location of office supporting project; and (7) Ability to produce all plan documents on a CAD system fully compatible with AutoCad 2004. A/E firms which meet the requirements listed in this announcement are invited to submit four (4) copies of the Standard Form 330. NO BID PACKAGES ARE AVAILABLE FOR THIS PROJECT. Each offeror shall identify past or current contract/agreements for efforts similar to the requirements stated. The past performance information can include, but is not restricted to, performance information regarding predecessor companies, key personnel who have relevant experience, or subcontractors that will be performing major or critical aspects of the requirement. In the case of an offeror without a record of relevant past performance, or for who information on past performance is not available, the offeror may not be evaluated favorably or unfavorably on past performance. The area of consideration will be limited to the firms within a mileage radius of 200 miles. North American Industry Classificatoin System (NAICS) code is 541330. The size standard is $4.5 million.
