Description
The intent of this contract is for the contractor to provide for the delivery and management of primary and preventive medical care and the continuity of that care for all enrolled Veterans Integrated Service Network 11Department of Veteran's Affairs patients at a health care facility centrally located in the vicinity of Yale, Michigan to serve veterans. The contractor may have one or more sub-contractors to provide specific portions of the care within the contract. b. The contractor shall provide health care providers, medical facilities, medical equipment and supplies, short-term pharmaceutical prescriptions and medical supplies (i.e. insulin, insulin syringes, Advantage test strips, dressing changes), and all administrative functions to fulfill the level of care and range of services necessary to meet the primary medical needs of the enrolled patients. c. The contractor shall be responsible for the administration and management of all aspects of this contract and the health plan covered there under. This includes responsibility for all contractor employees, subcontracts, agents, and anyone acting for or on behalf of the contractor. d. The term of the contract will be one year with 4 one-year renewable options. The effective date will be the written Notice to Proceed. e. Within ten (10) calendar days of contract award, the contractor shall provide the name and title of the contractor employee authorized to approve contract modifications. The contractor and the Detroit VA Medical Center shall agree on a schedule for incrementally enrolling patients within the first year. It is estimated that between 2800-3000 patients may be assigned to the contractor's plan during the first year. g. The average estimated age for the population in the area anticipated to utilize a VA community based outpatient clinic in the vicinity of Yale, MI is 67 years. The target population is 97% male.
