Description
This notice is a combined synopsis/solicitation to procure commercial items prepared in accordance with the format in the Federal Acquisition Regulations (FAR) subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is NHLBI-PB(HG)-2007-013-KLW, and the solicitation is issued as a request for quotation (RFQ). The solicitation/contract will include all applicable provisions and clauses in effect through the Federal Acquisition Circular 97-15. The acquisition is being conducted as a competitive, total small business set aside. The North American Industry Classification System (NAICS) code for this procurement is 493120 and the size standard is $23.5 M. The National Heart, Lung and Blood Institute (NHLBI), Office of Acquisitions (OA), on behalf of The National Human Genome Research Institute (NHGRI) is conducting the solicitation to provide Bio-repository Storage Services and Packaging Materials for a period of twelve (12) months. This service is needed to store serum and plasma at -80?? C for biological samples collected from over 1,000 clinical research patients. These samples will be used for sequencing and other testing. It is estimated that the repository services will require the following: 1) Two (2) each, Ultra-Low Upright Freezers, -70?? C to -85?? C, ??21 cu ft. 2) Two (2) each, Liquid Nitrogen Freezers, Non-Segregated Vial Box Storage, -135?? C to -170?? C, 21-35 2ʺ Vial Boxes (per box rate). 3) Three Hundred Sixty One (361), Packaging/Materials, 5??5??2 freezer boxes including 9??9 and/or 10??10 inserts for 1 ml vials or smaller. 4) Storage services shall include: one (1) shipment each week from the Contractor??s facility; one (1) shipment received each week from the Government; inventory services which includes the use of barcode technology to be confirmed prior to beginning inventory of material; comprehensive monthly reports summarizing temperatures and all inventory transactions; automatic insurance of $10,000.00 per contract; full access during business hours for authorized personnel; full repair services on all Customer owned equipment placed within the repository; and a complete audit of Contractor??s standard operating procedures. The proposed period of performance is March 2007 through March 2008. With the option to extend the period of performance for two (2) additional twelve (12) month periods. The F.O.B. Point is Origin (Contractor??s Facility). The provisions of FAR Clause 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition and a statement regarding any addenda to the provision. The provisions of FAR Clause 52.212-2, Evaluation - Commercial Items is applicable to this acquisition. The award will be made to the lowest priced, technically acceptable offeror meeting the Government??s specifications. The quote should address the Offeror??s understanding of the technical requirements and specifications of the equipment; qualifications of personnel; and provide evidence of successful past performance provide similar or equivalent services. The offeror must include a completed copy of the provision of FAR Clause 52.212-3, Offeror Representations and Certifications - Commercial Items with its offer. The provisions of FAR Clause 52.212-4, Contract Terms and Conditions - Commercial Items, applies to this acquisition. The FAR Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items applies to this acquisition. Interested small business vendors capable of furnishing the government with the products and services specified in this notice should submit a quotation to the address below. The quotation shall include the itemized price, commercial price list, prompt payment discount terms, the F.O.B. Point(Destination or Origin) shipping cost, if applicable, business size, taxpayer identification number (TIN), Dunn & Bradstreet Number (DUNS) and descriptive literature of the product quoted. The offeror must also register in the Government??s Contractor Registry System, note: www.ccr.gov. Quotations are due ten (10) calendar days from the publication date of this notice or March 21, 2007, 3:00 p.m. local time, which ever is later. The quotation must reference solicitation number NHLBI-PB-HG-2007-013-KLW. All quotations submitted in response to this announcement, referencing synopsis number NHLBI-PB-HG-2007-013-KLW may be submitted to the National Heart, Lung, and Blood Institute, Office of Acquisitions, Procurement Branch, 6701 Rockledge Drive, Suite 6042, Bethesda, MD 20892-7902, Attention: Kesha Williams or to williamk@nhlbi.nih.gov.
