- This announcement is a combined synopsis/solicitation prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation # is CHR7001 and is a Request for Quote incorporating provisions and clauses in effect through FAC 2005-13 and NAICS code 561612. The contract line items are as follows: CLIN0001 ? Base Year for Drug Testing during the hours 8:00am to 4:30 pm for 12 months in accordance with the attached statement of work; CLIN 0001AA - Collection of Drug Test Specimens On-Site at USUHS, qty. 110; CLIN 0001AB ? Laboratory Analysis of each Specimen, Qty. 110; CLIN 0001AC ? Blind Proficiency Specimens at the rate of 10% of all drug tests. Qty.10; CLIN 0001AD ?
