Description
Scope of Work Radiation Oncology Physics Services Radiation Oncologist: The Radiation Oncologist at the VAMC Northport is Steven Carabell M.D. He can be contacted at (631) 261-4400 ext 2375. Normal Business Hours: Monday - Friday, 8:00 am - 4:30 pm, excluding Federal Holidays. Location: VAMC Northport 79 Middleville Rd. Northport, New York 11768 Radiation Oncology, Bldg-200, Clinical Addition, Basement, Room D044 Equipment: Varian model Clinac iX Medical Linear Accelerator with following accessories: a. Dual photon & multiple electron Energies b. Multileaf Collimator c. Digital Portal Imager d. IMRT Capability e. Patient Positioning Lasers f. Eclipse IMX 3D Integrated Treatment Planning System g. Portal Dosimetry Software h. Respiratory Gating System i. ARIA Record and Verify System j. Life Line RadCalc QA Software k. EPID Phantom & Software l. Unidose Electrometer m.Linacheck Daily QA Meter Task: The Contractor shall supply all Travel, Materials, Test Equipment and Services necessary to perform all phases of Physics Services for the Radiation Oncology Service for up to twenty (20) patients at a time. General Requirements: The contractor shall: Review/complete the on-line VAMC Contract Worker Orientation Handbook/Post-test and abide by all facility rules/regulations. (http://vaww.northport.med.va.gov/educationnew.htm) Comply with all OSHA requirements as related to Performance of Work. Obtain Contractor Photo Identification Badges from VAMC Police Dept. Furnish, Wear and Maintain their own Personnel Dosimeters. Complete Varian courses (Las Vegas, on site, and web based) or submit documentation of training by Varian for Clinac iX, Eclipse, Aria, and MLC. (Travel provided by Varian as part of VA Purchase Order). Equipment Quality Assurance: The contractor shall maintain a Quality Assurance Program for the linear accelerator as required by New York State following the American Association of Physicists in Medicine's TG40 Protocol. This includes the following monthly tests. X-ray output constancy. Backup monitor constancy. X-ray central axis dosimetry constancy. X-ray beam flatness constancy. X-ray symmetry. Emergency off switches. Wedge interlocks. Light/radiation field coincidence. Gantry/collimator angel indicators. Field size indicators. Cross-hair centering. Treatment couch position indicators. Provide annual Linac calibrations as required or after major repair. This testing includes all the monthly tests and the following additional tests: X-ray output calibration constancy. Field size dependence of x-ray constancy. Off-axis factor constancy. Transmission factor constancy for all treatment accessories. Monitor chamber linearity. X-ray output constancy vs. gantry angle. Arc mode. Safety interlocks. Collimator rotation isocenter. Gantry rotation isocenter. Couch rotation isocenter. Coincidence of collimator, gantry, couch axes with isocenter. Coincidence of radiation and mechanical isocenter. Table top sag. Vertical travel of table. Check all patient charts for prescription, set-up, and calculation errors against computerized treatment plans. Provide charts for easy hand calculations of monitor units to a specific point. In-service and monitor the Radiation Therapist in all QA procedures and treatment planning routines. Review and update all radiation safety procedures. Assistance as required in emergencies. Audit entire Radiation Therapy Program on a periodic basis and submit reports. Perform QA testing, as required by New York State on the following equipment. Simulator, following AAPM's TG40 guidelines. Treatment Planning System. Portal Imaging. Respiratory Gating. Multi-Leaf Collimators. Provide dosimetry support, including: Generating computerized treatment plans. Consulting with physicians and therapists to ensure accurate and reproducible treatments. Perform QA on IMRT treatment plans. Timeframe: All work performed outside of Normal Business Hours shall be coordinated/approved in advance with the Therapist or Physician. Experience: The contractor shall be an Independent/Certified/Licensed Health Physicist. The contractor shall have an insured, active, legitimate business in Medical Physics and shall demonstrate practical experience working with this Equipment by providing the VAMC Northport with a minimum of three (3) references. Test Equipment: The contractor shall provide and utilize only fully calibrated test equipment /measurement instruments to perform the Task and shall furnish copies of calibration certificates. Documentation: The contractor shall provide the Radiation Oncology Service with documentation of all Tasks. Justification This equipment provides Radiation Therapy for Oncology Patients. It is utilized by specialized trained staff to administer a predetermined course of radiation treatment doses which subsequently destroy cancer cells/tumors. An injurious or lethal radiation dose potentially may be administered to the patient if this equipment is not in full compliance with New York State Department of Health Regulations or not calibrated to Original Manufacturer's Specifications. Quality Assurance, Clinical Consultation and Treatment Planning by a Qualified Medical Physicist is essential for the functioning of a Radiation Oncology Service.
