- The Department of Veterans Affairs Louis Stokes Cleveland VA Medical Center, Cleveland, Ohio is in need of a contractor(s) to provide Advanced Life Support (ALS) and Basic Life Support (BLS) ambulance services for eligible beneficiaries of the VA. The Contracting Officer anticipates making up to eight separate awards which shall cover eight geographic zones. Medical facilities plus counties included in each geographic zone follow: (1) ZONE 1 includes Sandusky CBOC and counties Ottawa, Sandusky, Seneca, Erie, Huron; ZONE 2 includes Cleveland VA Medical Center (both the Wade Park unit and the Brecksville unit), the Lorain CBOC, McCafferty CBOC and counties Lorain, Cuyahoga, Medina; ZONE 3 Painesville CBOC and counties Lake, Geauga, Ashtabula; ZONE 4 Mansfield CBOC, Wooster CBOC and counties Richland, Ashland, Wayne; ZONE 5 Akron CBOC, Canton CBOC, Ravenna CBOC and counties Summit, Portage, and Stark; ZONE 6 Warren CBOC, Youngstown CBOC and counties Trumbull, Mahoning; ZONE 7 New Philadelphia CBOC and counties Holmes and Tuscarawas; ZONE 8 East Liverpool/Calcutta CBOC and counties Carroll and Columbiana. Period of performance shall be a base period plus 4 option years. This solicitation is being issued as a 100 percent small business set-aside. The NAICS code for this procurement is 621910 and the size standard is $6.5M. All offerors shall be registered with the CCR database at www.ccr.gov prior to award of any contract.
