- Description
- The VA Illiana Health Care System, Danville, Illinois has a requirement for an Indefinite Delivery/Indefinite Quantity contract for the VA Outpatient Clinic, West Lafayette, Indiana back-up (routine & STAT) laboratory services. VA intends to award a base & four option year contract. Contract shall be for a Base Period beginning from date of contract award through September 30, 2007, with four option years through September 30, 2011 with services provided at contractor's site. Contractor shall be physically located in West Lafayette, Indiana, Illinois in order for veterans to travel from VA Clinic to contractor's site for lab services. Contractor shall provide laboratory services for any veteran who presents an approved request from any VA OPC-authorized physician and/or for any specimen provided by VA to be processed. The Government reserves the right to provide phlebotomy services & provide specimens only to the contractor for processing. The Government reserves the right to provide phlebotomy services and process routine laboratory tests in-house at its parent site, VA Illiana Health Care System, Danville, Illinois. Contractor shall furnish all management, supervision, labor, equipment, supplies, and transportation necessary to perform requested laboratory services. The Contractor shall be approved to provide clinical diagnostic laboratory tests under both Medicare & Medicaid regulations & in accordance with all federal and state law. Contractor's clinical laboratory shall be fully accreditated by College of American Pathologists (CAP), Department of Health & Human Services HCFA Clinical Laboratory Improvement Amendment (CLIA), and the Joint Commission on the Accreditation of Hospitals (JCAHO) to be considered for award. Contractor personnel shall be currently licensed and shall maintain a full & unrestricted license at all times in a state, territory, or commonwealth of the United States. NAICS code is 621511. The Small Business Size Standard is $12.5 million average annual receipts for the preceding three years. This is an unrestricted full and open competition. This procurement will be conducted in accordance with FAR 12 and FAR Part 13. Solicitation documents will be available on May 8, 2007. Offers will be due on or before May 24, 2007 at 4:15 P.M. (CST).
