- Contractor is to provide all space, personnel, equipment, materials, supervision, and professional and technical services necessary to provide Breast Care Services on a cost per procedure basis for the beneficiaries of the Tomah VA Medical Center, 500 East Veterans Street, Tomah, WI 54660, in accordance with the pricing, specifications, terms and conditions listed herein. The period of performance shall be from the date of award through May 31, 2008, plus four (4) one-year (1) options to renew, at the discretion of the Government. All work performed under this contract will be completed at the contractor's facility. This resulting contract from this solicitation will cover breast care services mainly for veterans residing in Central Wisconsin. Therefore, to minimize travel distances for veterans, the Contractor's facility providing services must be physically located within any of the following Wisconsin Counties: Adams, Clark, Marathon, Portage, Price, Taylor, Waupaca, Waushara, or Wood. This procurement is unrestricted (open to all vendors - regardless of size). North American Industry Classification System (NAICS) Code is 621512, size standard $12.5 million applies to this solicitation. Offerors are reminded to complete all sections of the solicitation, including the Offeror Representations and Certification-Commercial Items both through the ORCA website and with the solicitation.
