Description
This synopsis announcement is for VISN 1 pharmacy inventory as proscribed herein and in solicitation VA523-07-RP-0021 at the sites listed below. Inventory shall include all medication products purchased, with those directly purchase products with a total dollar value of $500.00 or more per line item to be excluded. The inventory is further limited to medications and supplies that are dispensed to patients. Items such as equipment, medical supplies and prescription labels and vials will be excluded. Said inventory is limited to areas that are under the direct control of each pharmacy. The contractor must be capable of providing an Excel spreadsheet with at least these data elements: a). Item name (name and strength, i.e., Ativan 1mg/1ml); b) Package description (ml, bottle, tab); c) NDC; d) UPC/UPN; e) Total Quantity; f). Current package price; g). Total replacement cost per item per site; h), Total replacement cost of all items per site. Said services are to be performed at the following Medical Center sites: Jamaica Plain, 150 South Huntington Ave, Boston, MA 02130; West Roxbury, 1400 VFW Parkway, West Roxbury, MA 02132; Brockton, 940 Belmont Street, Brockton, MA 02301; Boston OPC, 251 Causeway Street, Boston, MA 02110; Lowell OPC, 130 Marshall Road, Lowell, MA 01852; Worcester OPC, 605 Lincoln Street, Worcester, MA 01605. Site visits will be made available only for those who have requested and received a solicitation package. All vendors must be registered with Dunn & Bradstreet and with the federal government's Central Contractor Registry (www.ccr.gov) to be eligible to receive solicitations. This is a Small Business Set-Aside acquisition.
