Description: To conduct two phased mail and phone survey of a pre-selected targeted audience members of gulf-deployed and gulf-non-deployed military personnel for future Neuro-cognition issues, as part of a funded research project. The contemplated work herein is part of an on-going VA and DOD-funded research study. All the text in the materials sent to participants has been scripted by the VA study directors and has received appropriate IRB approvals. Survey shall include Phase One 269 surveys with cover letters, questionnaires, a business-reply envelope, and a follow-up postage-paid reply postcard, to the most recent address for target audience member, and follow-up telephone interviews. Phase Two: Contractor shall perform the following: mail 53 surveys with cover letters, questionnaires, a business-reply envelope, and a follow-up postage-paid reply postcard, to the most recent address for target audience member and follow-up telephone interviews. All surveys and phone surveys will be conducted within 120 days of award of the contract. It is the intent of the government to negotiate a sole source contract with Schulman, Ronca & Bucuvalas, Inc. to perform said services.
