Description
1. Amendment 0001 is hereby incorporated into the Request for Quote (RFQ) number W912JF-07-T-0011 and shall become a part of any resultant contract. 2. Pictures of the dinning facility are now posted at http://www.nationalguardcontracting.org/Ebs/AdvertisedSolicitations.asp?SolType=Simplified%20. 3. The questions and answers were either submitted prior to or during the site visit. Question: What type of serving plate is required? Answer: As stated in the statement of work, the Government requires a three compartment plate be used, no Styrofoam plates. Styrofoam plates are not sturdy enough to use. Question: Has the Government contracted for this type service before in the dinning facility located at Fort Chaffee? Answer: Yes, however the recent contracts have been on a smaller scale or provided partial support. In January 2007, there was a Contract awarded for $7,695. Contractor prepared and served 550 meals each for one breakfast and one dinner meal, respective ly. Contractor also provided food subsistence. In May 2006, there was contract awarded for $33,356. Contract was for cooks only. Contractor provided six cooks per meal period (breakfast and dinner) for 11 days. Question: Will the short order breakfast cook be required to prepare omelets? Can the cook prepare the omelets in advance? Answer: Short order cook will prepare fried eggs only and will prepare them as requested. Question: How much time will the contractor have to clean up after the final meal? Answer: As specified in the SOW, Contractor has up to two days. Contractor does not have to use the full two days to clean-up. Question: What kind of uniforms are required? Answer: Uniforms as specified in the SOW. Scrubs or hospital uniforms are acceptable. Employees should wear hat or hairnet. Question: How will the delivery of the raw subsistence be handled? Answer: As stated in the SOW, contractor is responsible for unloading supplies. However, Government will provide fork-lift and driver to remove pallets from delivery trucks. Government will provide assistance with breaking down pallets, when personnel are available. There is a storage room capable of holding a three day supply of dry goods. There are also two walk-in refrigerators and a walk-in freezer. Question: Can the contractor substitute or make additions to the menu? What happens to the leftovers? Answer: Contractor may make suggestions and requests for additional subsistence or spices to improve the quality of the meal. There is nothing in place for leftovers at time. Leftovers will be thrown away or referred to the Government COR or QAE. Question: How does the Government want the serving lines handled? Will there be a salad bar? Answer: There will be four serving lines. It is anticipated that only one or two lines will be needed until June 15th. Contractor shall rotate meals through all the serving lines prior to June 15th. This will insure that all lines are fully operational. Based on the meal schedule, all lines, including a short order cook for breakfast, will need to be open beginning June 16th. There are two salad bars in the facility. Question: What is the serving capacity of the insulated food containers for mermite meals? Answer: 90-100 servings. Question: Does the kitchen have air conditioning? Answer: No, there are some fans in the facility. Government will try to provide fans, however we would recommend bringing fans. Question: Can the contractor bring his/her own trailer and equipment? Answer:Yes,however_equipment must be removed when the contract is completed. Note: Government will not be responsible for contractors equipment. Question: Is the contractor required to provide eating utensils (knife, fork, or spoon)? Answer: Yes as stated in the SOW. Contractor shall also provide salad bowls. Note: Contactor should be washing primarily pots, pans, and serv i ng utensils. Patrons will police themselves by picking up their own trash and throwing it away on their way out. Question: How does contracting want the quotes submitted? Answer: In accordance to the RFQ. However, it is recommended that the contactor call the contracting office and verify the receipt of the quote. Question: Is there a place where used cooking oil can be discarded? Answer: Yes. Government shall have a company provide a container for the purpose of disposing the used oil. 4. The closing date for submission of offers will remain as May 1, 2007 at 11:00 AM CST.
