- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes ar e being requested and a written solicitation will not be issued. The solicitation number is W91YTV-07-T-S001. This acquisition is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-05. The North American Industry Classification System Code (NAICS) for this procurement is: 561310; the Standard Industrial Classification Code is 7363; and the small business size standard is $6.5M . This requirement is Unrestricted. Moncrief Army Community Hospital, Fort Jackson, SC 29207-5720, has a requirement for the following: Provide services to assist the optical laboratory in the production of glasses and optical devices. UNIT PRICE $__________, TOTAL CLIN $__________. The government anticipates awarding a single award for this requirement for the period 1 June 2007 to 30 September 2007.
