Description: SOURCES SOUGHT NOTICE: DME and HME Pickup, Delivery, Storage and Repair Services Contract. The Department of Veterans Affairs is currently seeking sources of services. This is a sources sought synopsis only. This is not a solicitation announcement. No contract will be awarded from this notice. No reimbursement will be made for any cost associated with providing information in response to this notice. The proposed requirement mentioned herein is being considered for Service Disabled Veteran-Owned Small Businesses; Small Businesses; Small Disadvantaged Businesses, 8 (a); Women Owned Businesses; Veteran-Owned Small Businesses; Hub-zone certified; potential joint venture partners; or teaming partners. The government will use responses to this sources sought notice to make appropriate acquisition decisions. The PWS falls under the North American Industry Classification System (NAICS) code 446199. Interested firms must demonstrate in their qualifications statement that they are qualified to perform all of the PWS tasks under this NAICS code. The resultant contract is anticipated to be Fixed Rate Indefinite Delivery Indefinite Quantity with a twelve (12) month base period and the possibility of four twelve (12) month award terms. The contractor will be reimbursed for other direct costs incurred during the period of performance of the contract. Work will be ordered through the issuance of purchase orders. Contractor will provide to the VA Salt Lake City Health Care System storage, delivery, assembly, set-up, instruction, pick-up, sanitation, disposal, minor repair for reissue of durable and home medical equipment. Deliveries will be made as such times and in such quantities as requested, for (first) newly purchased DME and HME as specified in writing on purchase orders (POs) authorizing said services by the Prosthetics Section, then (second), delivery of VA property in storage. Quality of services and support should meet or exceed JCAHO standards. The covered area includes the state of Utah; Elko and White Pine counties of Nevada; Clark, Fremont, Jefferson, Madison, Teton, Bonneville, Bingham, Power, Bannock, Caribou, Oneida, Franklin and Bear Lake counties of Idaho; and Teton, Sublett, Lincoln, Uinta and Sweetwater counties of Wyoming. Interested Service Disabled Veteran-Owned Small Businesses; Small Businesses; Small Disadvantaged Businesses, 8 (a); Women Owned Businesses; Veteran-Owned Small Businesses; Hub-zone certified; potential joint venture partners; or teaming partners should provide the VA with appropriate documentation (i.e., a detailed capability statement and/or relevant past performance history documenting its ability to perform similar requirements). Standard company brochures are not desired. Interested concerns who believe they have the capability to provide the required services are invited to submit two (2) copies of a Qualifications Statement. Responses should also include the following information: company name, address, point of contact, phone number and email address; Small Business or Small Disadvantaged Business status; 8 (a); Women Owned Businesses; Service Disabled Veteran-Owned Small Business; Veteran-Owned Small Businesses; Hub-zone certified status; potential joint venture partners or teaming partners. No feedback or evaluations will be provided regarding the information provided. This Sources Sought notice neither constitutes a Request for Quote, nor does it restrict the government to an ultimate acquisition approach. This Sources Sought notice should not be construed as a commitment by the government for any purpose.
