Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quote, solicitation number W91NZ6-7038-0004. The South Dakota National has a requirement for Dental Services from 07:00AM  04:00PM during 2729 June 2007 at the armory located in Sioux Falls, SD. SALIENT CHARACTERISTICS ASSOCIATED WITH THIS ACQUISITION ARE: 1. Contractor must be able to provide annual exams to include required digital x-rays (Digital Bite Wing and Panoramic X-ray system) for 205 soldiers and must be OSHA Compliant. 2. Contractor must classify soldiers IAW the following criteria listed in the below statement of work. a) STATEMENT OF WORK Dental Work The Medical Readiness Branch of the xx National Guard requires soldiers assigned to designated units, and identified as being within Dental Classification 3, to have necessary dental treatment initiated to bring them up to Dental Classification Standard s 2, as discussed below: a. CLASS 1: Patients not requiring dental treatment or reevaluation within 12 months. Criteria are- (1) No dental caries or defective restorations (2) Arrested caries for which treatment is not indicated (3) Healthy periodontium, no bleeding on probing; oral proxphylaxis not indicated. (4) Replacement of missing teeth not indicated. (5) Unerupted, partially erupted, or malposed teeth that are without historical, clinical, or radiographic signs or symptoms of pathosis and are not recommended for prophylactic removal. b. CLASS 2: Patients who have oral conditions if not treated or followed up, have the potential but are not expected to result in dental emergencies within 12 months. Criteria are- (1) Treatment or follow up indicated for dental caries with minimal extension into dentin or minor defective restorations easily maintained by the patient where the condition does not cause definitive symptoms. (2) Interim restorations or prostheses that can be maintained by the patient for a twelve month period. This includes teeth that have been restored with permanent restorative materials but for which protective coverage is indicated. (3) Edentulous areas requiring prostheses but not on an immediate basis. (4) Peridontal disease or periodontium exhibiting: (a) Requirement for oral proxphylaxis (b) Requirement for maintenance therapy; this includes stable or non-progressive mucogingival conditions requiring periodic evaluation. (c) Non-specific gingivitis. (d) Early or mid adult periodontitis. (5) Unerupted, partially erupted, or malposed teeth that are without historical, clinical, or radiographic signs or symptoms of pathosis, but which are recommended for prophylactic removal. (6) Active orthodontic treatment. (7) Temporomandibular disorder patients in maintenance therapy. CLASS 3: Patients who have oral conditions that if not treated are expected to result in dental emergencies within 12 months. Patients should be placed in class 3 when there are questions in determining classification between Class 2 and Class 3. Criteria are- (1) Dental caries, tooth fractures, or defective restorations where the condition extends beyond the dentinoenamel junction and causes definitive symptoms; dental caries with moderate or advanced extension into dentin; and defective restorations not mainta ined by the patient. (2) Interim restorations or prostheses that cannot be maintained for a 12-Month period. This includes teeth that have been restored with permanent restorative materials but for which protective coverage is indicated. (3) Periodontal diseases or periodontium exhibiting: (a) Acute gingivitis or pericoronitis. (b) Active moderate to advanced periodonitis. (c) Periodontal abscess . (d) Progressive mucogingival condition. (e) Periodontal manifestations of systemic disease or hormonal disturbances. (4) Edentulous areas or teeth requiring immediate prosthodontic treatment for adequate mastication, communication, or acceptable esthetics. (5) Unerupted, partially erupted, or malposed teeth with historical, clinical, or radiographic signs or symptoms of pathosis that are recommended for removal. (6) Chronic oral infections or other pathological lesions including: (7) Pulpal or periapical pathology requiring treatment. (8) Lesions requiring biopsy or awaiting biopsy report. (9) Emergency situations requiring therapy to relieve pain, treat trauma, treat acute oral infections, or provide timely follow-up care (for example, drain or suture removal) until resolved. 3. Contractor is required to input applicable data in the DENCLASS module within the MEDPROS database system. 4. Contractor is responsible to provide dental readiness education for Soldiers IAW USCHPPM Standards; all electronic records must meet HIPPA standards. Standards can be viewed at (http://chppm-www.apgea.army.mil). 5. Contractor will be responsible to provide Dental restorative care for CAT 3 solders. a) Contractor will provide necessary treatment and will include patients name, SSN, SDARNG Unit of assignment and each ADA code to correspond with individual charges. Contractor must receive approval from SDARNG State Dental Officer prior to performi ng any restorative care. b) The Contractor will use the criteria 1-9 listed under class 3 in the Statement of Work. GENERAL INFORMATION: 1. There will be approximately 205 soldiers and we anticipate 25% of the soldiers will likely be in CAT 3. 2. Contractor will be required to provide services at site location, 801 W NATIONAL GUARD DR SIOUX FALLS SD 57104-0117. If the Offeror is not proposing the terms stated above, the Offeror will be eliminated. The Contractor will meet the Governments requirements. Offerors shall provide a per soldier price for salient characteristics 1, 2, 3 and 4. In addition the offeror shall include the predicted number of exam staffing, treatment staffing, number of Dentists, RDAs and Project Managers for the requested agenda in the contract. This notice incorporates provisions and clauses in effect through the Federal Acquisition Circular 2005-15. The government intends to award a Firm-Fixed Price contract resulting from this notice on an all o r none basis to the responsible offeror whose offer meets the following requirements: Dental License, proper equipment, service time frame and meets the stated requirements stated above.
