Description

DENTAL LAB Statement of Work(LOCAL) Provide Dental Laboratory prosthetics for removable complete and partial dentures, and for processed night guards. Removable Prosthetics Maxillary and mandibular complete dentures Maxillary and mandibular partial dentures Processed Night Guards Repairs Complete Dentures Partial Dentures The Dental Laboratory Contractor shall provide services during the following time period. Monday - Friday 8:00am - 4:30pm Pick-Up Items and Turn Around Times The Dental Service will contact Dental Laboratory when lab prostheses are ready to go to the Dental Laboratory. The Contract Dental Laboratory will pick-up and deliver prosthetic items within ten working days of pick-up. The VAMC Dental Service Dentist will fill out Laboratory Rx and Contract Dental Laboratory will follow their instructions. Contractor must provide telephone numbers for means of communications to permit immediate inquiries regarding any questions the Dentists may have concerning dental prosthetic items. The Contractor Laboratory must be within ten miles of the VAMC Manchester to ensure speedy pick-up and delivery. Quality Control The Dental Service has the right at any time to terminate the contract if the quality of work provided has less than acceptable results for our veteran patients.
