Description
The Federal Bureau of Investigation (FBI) has a requirement to provide complete physical examinations, utilizing board-certified physicians in the Washington, DC and Houston, Tx locations in accordance with the requirements stated below: The requirements will be in the following areas: (1) Board-Certified Physicians in any of the following: cardiology, emergency medicine, internal medicine, family medicine, or preventive medicine (aerospace, occupational, or general preventive medicine and public health) to qualify for performance of physical examinations, (2) Applicant Physical Requirements, (3)Complete Medical History, (4) Hands-on Examinations, (5) Documentation and Findings, (6) Fitness For Duty, (FFD) Periodic Exams, (On-Board Employees, (7) Laboratory Tests, (8) FFD Physical Examination tasks to be performed by physicians, (9) Hands-on Examinations, (10) Site Visit Evaluations. All proposed medical personnel shall be subject to a background check, to be performed by the FBI. The Government intends to award an Indefinite Delivery Indefinite Quantity, Firm-Fixed-price contract with a base year and four, one-year option periods. Multiple awards may result from the award of this contract. The FBI reserves the right to make multiple awards from the single synopsis. The solicitation package is available upon requests. Offerors shall submit past performance information in their proposal. At a minimum, this shall consist of at least three most recent, prior customer point of contacts with telephone numbers for performance of similar experience. Technical qualifications, Price and Past Performance will be evaluated. All vendors must have registered in the Central Contractor Registration Database at www.ccr.gov to receive this award. The Proposals are due in sealed envelopes/packages no later than 1:00 p.m. Eastern Standard Time, May 29, 2007. Proposals shall be clearly marked/addressed as follow: FBI, Attn: Angela M.
