- SCOPE OF SERVICES: CONTRACTOR shall provide Physical/OccupationalTherapy Services described in the Statement of Work to Veterans of the Department of Veterans Affairs Medical Center (VAMC), Providence, RI. in accordance with the attached terms and conditions of the contract and specifications. Contractor shall provide one or more physical therapists; physical therapist assistants; or occupational therapists up to 40 hours per week on an "as needed" basis. DUTIES/RESPONSIBILITIES: 1. PHYSICAL THERAPIST DUTIES AND RESPONSIBLITIES Screens referred patients to determine the need for intervention; Evaluates patients using accepted testing practices, develops treatment plans and implements the intervention plans directly or in collaboration with others, adapting environment, equipment, devices, activities and exercises according to the needs of the individual and their cultural context; Observes patient response to interventions, reviews the interim progress of Patients toward their rehabilitation goals and modifies the treatment plan as indicated, terminating services when maximum benefit is achieved and formulating discharge and follow-up plans; Documents Physical Therapy evaluation, treatment and outcomes as outlined in Physical Medicine and Rehabilitation Section policies. Collaborates with patients, families and members of the hospital staff to ensure the patient achieves his/her maximum rehabilitation potential; Recommends home visits and family meetings when indicated. PHYSICAL THERAPIST ASSISTANT DUTIES AND RESPONSIBILITIES: a) Responsible for independently carrying out specialist and diverse treatment for physically disabled patients, astute observation of warning signs and/or problems requiring timely intervention and is knowledgeable in modifying treatment program and/or modalities to obtain maximum patient progress; b) Responsible for patient/family education to include ongoing verbal instruction, referral to self-help groups as appropriate, issuing printed material and establishing therapeutic home programs; Responsible for all other duties normally performed by a VA Physical Therapist Assistant at VAMC. 3. OCCUPATIONAL THERAPIST DUTIES AND RESPONSIBILITIES: a) Responsible for responding to inpatient consults. This process would include collecting the new consults from the office; b) Review appropriate notes in EPN; c) Review patient information with nursing staff; d) Evaluate patient by acceptable OT guidelines; e) Formulate a written treatment plan and document in EPN; f) Coordinate treatment and planning with appropriate OT and interdisciplinary staff; g) Oversee ordering of proper DME when indicated; h) Oversee discharge goal setting with patient pertaining to community follow up; i) Provide input for interdisciplinary discharge planning meetings held twice weekly. j) Assist with outpatient consultations during decreased inpatient workload times. The process for responding to outpatient consults requires evaluation by OT standards, formulation of a treatment plan to include goal setting, and provide or coordinate treatment and provision of equipment. 4. CONFIDENTIALITY: Under this contract, CONTRACTOR is subject to the provisions of the Privacy Act of 1974, as well as all applicable VA laws and regulations. Physical Therapists/Physical Therapist Assistants/Occupational Therapists employed by CONTRACTOR shall keep confidential the contents of all medical documents entrusted to its care. Contractor shall sign the Department of Veterans Affairs Business Associates Agreement (BAA) agreeing to Health Insurance Portability and Accountability Act of 1996 (HIPAA) compliance. CONTRACTOR employees shall not disclose to any unauthorized person, firm or corporation knowledge entrusted during the performance of this contract, which relates to the following: Names of patients Names of physicians, nurses, therapists, or any other employee of the VA The type or content of any dictated reports 5. SCHEDULING: Scheduling will be determined by the Chief of Physical Medicine & Rehabilitation Service (herein after called PMRS) for the services required by this contract. 6. MONITORING PROCEDURES: The services provided by physical therapists/assistants/occupational therapists of the CONTRACTOR will be under the direction of the VA's Chief PMRS. He/She shall Perform the duties of the Contracting Officer's Technical Representative(COTR) in directing the activities under the contract. The COTR is responsible for documenting the CONTRACTOR's attendance and actual services rendered through the establishment and maintenance of reliable record keeping system, which will be the mechanism for certifying the invoices for payment. Employees of CONTRACTOR must be present at the VA Medical Center facility and must be actually performing the required services for payment. 7. QUALIFICATIONS: CONTRACTOR's Physical/Occupational Therapists assigned to perform the services covered by this Contract shall be licensed in a State, Territory, Commonwealth of the United States or the District of Columbia. Physical/Occupational Therapist (s)/assistant (s) must possess a current, unrestricted Physical/Occupational Therapist license. Be credentialed and privileged by the VA to practice physical/occupational therapy. JCAHO REQUIREMENTS: Although this contract does not require JCAHO accreditation or other regulatory requirements regarding worker competency, employees of CONTRACTOR must perform the work in accordance with JCAHO standards. CONTRACTOR shall be responsible to ensure that all of its employees providing services under this contract are fully trained and completely competent to perform the required services. WORK HOURS: The services covered by this contract shall be furnished by CONTRACTOR on a scheduled or as needed basis. Normal work hours for the Physical Therapy Clinic are 8:00 AM to 4:30 PM (Monday thru Friday). The clinic will be closed on all VA Holidays: (New Years day, Martin Luther King Day, George Washington's Birthday, Memorial Day, 4th of July, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day.) CREDENTIALING AND PRIVILEGING: Upon award of this contract and before performing any services, CONTRACTOR's employees, who are regarded as licensed independent physical therapists, must be approved for credentialing and granted privileges by the VA's Physical Therapy Office in accordance with VHA DIRECTIVE 2006-067. During the term of the contract, CONTRACTOR must notify the Contracting Officer in writing of any anticipated change of its employees to be performing services and furnish all data necessary for credentialing requirements, prior to providing contract services. MEDICAL INSURANCE: CONTRACTOR must show proof of medical malpractice insurance prior to award of this contract. See VAAR clause 852.237-2, Indemnification and Medical Liability Insurance. PERSONNEL POLICY: CONTRACTOR shall be responsible for protecting the personnel furnishing Services under this contract. CONTRACTOR shall provide the following: Workers Compensation Professional liability insurance Income tax withholding Social Security payments CONTRACTOR shall provide all wages and compensation that are due and owing to Employees performing work under this contract. Payment for any leave, including sick leave or vacation time, and holiday pay is the responsibility of CONTRACTOR. CONTRACT MODIFICATIONS: This contract may be modified only by agreement of both parties. All modifications to the contract must be in writing and approved by the Contracting Officer. 14. VA POLICIES AND PROCEDURES: The services to be performed shall be in accordance with VA policies and procedures, and the regulations of the Medical staff by-laws of the VA facility. 15. OTHER TERMS AND CONDITIONS: VAMC reserves the right to perform any portion of this contract by its own qualified personnel at any time during the term of the contract. All therapists will be required to undergo a low-risk background investigation in accordance with the schedule, terms and conditions of the contract. Offers will be evaluated for Technical Capabilities (Contract Staffing and Oversight), Past Performance, and Price. Technical capabilities and Past Performance, when combined, are approximately equal to price. Award will be made to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered.
