- This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; pro posals are being requested and a written solicitation will not be issued. Fort Huachuca, AZ is soliciting proposals for the following requirement: The Contractor shall furnish all personnel, labor, uniforms, training, materials, equipment, vehicles, superv ision and other items or services (except as specified in the PWS as Government furnished) required to manage and provide Advanced Life Support (ALS) ground emergency ambulance services on a 24-hour per day basis, as required for the service area of Fort H uachuca, AZ. This is a non-personal service contract that shall be provided in accordance with Arizona State Laws, Federal Laws, Army Regulations and the terms of this contract. Offerors must currently possess, or be able to obtain by award date, through the Arizona Department of Health Services, Bureau of Emergency Medical Services and Trauma System, a current Certificate of Necessity (CON) and must adhere to the restrictions of the CON and operate in accordance to the statutes and rules by which it is go verned. Desired performance start date for this contract is 3 June 2007, however, offerors can propose an earlier start date. Any Technical Exhibits and the Performance Work Statement detailing the area of responsibility and work performance requirements c an be requested from the Contract Specialist listed below. The NAICS code for this procurement is 621910 Ground Ambulance Service and the small business size standard is $6.5 million. FOB DESTINATION: Fort Huachuca, AZ. EVALUATION CRITERIA/PROCEDURES: The solicitation is issued as a request for proposal (RFP) and will be awarded as a firm-fixed-price contract. All proposals will be evaluated based on price, possession of Arizona CON (either by submission of current CON or copy of the CON application) and submission of an Ambulance Service Backup Plan. The plan will out line how the Contractor will continue operations in the event all available ambulances are in use, out of commission, or unavailable for any reason to maintain Advanced Life Support (ALS) ground emergency ambulance services on a 24-hour per day basis, as r equired for the service area of Fort Huachuca, AZ.
