- This is amendment 0001 of solicitation # W81K04-07-T-0002. The due date to submit proposals has been extended to Monday, 21 May 07 at 3:00 p.m. CST. FAR Clause 52.237-7, entitled Indemnification and Medical Liability Insurance, has been deleted. Price factors by location will be considered. 52.212-3 OFFEROR REPRESENTATIONS AND CERTIFICATIONS-COMMERCIAL ITEMS Implements a new representation and certification process that allows for representations and certifications to be submitted on line at http://orca.bpn.gov in efforts to eliminate the administrative burden for contractors of submitting the same information to various contracting offices and establish a common source for this information to procurement offices across the Government. LICENSE VERIFICATION AND CREDENTIALS REVIEW OR PRIVILEGING: The contractor will follow Army Regulation (AR) 40-68 Clinical Quality Management for all healthcare services provided in the MTFs. Army Regulation 40-68 is located on the internet at website http://www.usapa.army.mil/pdffiles/r40_68.pdf. The COR will d esignate the offices responsible for license verification for non-privileged personnel and the Credentials Office is responsible for credentialing and privileging actions. If the contractor is providing privileged health care providers, the contractor sha ll check with the Credentials Office to determine their credentialing/privileging process and timelines so that the health care providers can be privileged prior to the start of work. The contractor shall perform all primary source verifications and include all verifications with the credentials package or competency assessment folder information for non-privileged staff in accordance with AR40-68. The contractor shall obtain all paper work, letters of reference, approvals, work history, etc required by the MTF credentialing or licensure verification processes. The MTF staff will not assist the contractor in obtaining any needed information for initial credentialing. The contractor is responsible for the current competence of contract HCPs used to provide healthcare services under this contract. Licensure, certification, registration, or authorizing documents (license and certification verification processes chapter 4 of AR40-68). All personal services contract health care providers must maintain a current, active, valid and unrestricted license or authorizing document from any U.S. jurisdiction. In addition, HCPs who write prescriptions for controlled substances must have a current Drug Enforcement Agency (DEA) certificate or Controlled Drug Substance numbers as appropriate.
