- THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS (NON-PERSONAL SERVICES)PREPARED IN ACCORDANCE WITH THE FORMAT IN SUBPART 12.6 AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION. PROPOSALS ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. THIS SOLICITATION IS ISSUED AS A REQUEST FOR QUOTATION (RFQ). SUBMIT ONLY WRITTEN QUOTATIONS FOR RFQ FOS25N71410101. THIS PROCUREMENT IS UNRESTRICTED. THE ASSOCIATED NAICS CODE IS 611620. SPANGDAHLEM AIR BASE HAS A REQUIREMENT TO PROVIDE A YOUTH SOCCER CAMP AS OUTLINED IN THE STATEMENT OF WORK. PERFORMANCE WILL BE FROM 05 AUG 07 ? 11 AUG 07(GEILENKIRCHEN) AND 12 AUG 07 ? 18 AUG 07 (SPANGDAHLEM). THE GOVERNMENT WILL AWARD A PURCHASE ORDER RESULTING FROM THIS SOLICITATION TO THE RESPONSIBLE OFFEROR THAT IS MOST ADVANTAGEOUS TO THE GOVERNMENT. QUOTES MUST BE VAILD FOR 30 DAYS. THE FOLLOWING FACTORS WILL BE EVALUATED TECHNICAL CAPABILITY/PAST PERFORMANCE OF THE ITEM BEING PROPOSED, PRICE AND DELIVERY SCHEDULE IN COMPLIANCE WITH THE REQUIREMENTS THIS SOLICITATION WITH PRICE BEING THE MOST SIGNIFICANT FACTOR. PRICE QUOTATIONS SHALL INCLUDE ALL TRANSPORTATION COSTS ASSOCIATED WITH THE PERFORMANCE OF THE STATEMENT OF WORK. ***************************************************************** STATEMENT OF WORK (SOW)- YOUTH SOCCER PROGRAM 1. DESCRIPTION OF CONTRACTOR PROVIDED SERVICES: The contractor shall provide non-personal service to include all labor, materials, transportation, to staff and operate a weekly soccer program inclusive all travel costs to/from and within Europe, as outlined below: 1.1. Program Scope: - Travel to Geilenkirchen a USAFE installation, 5 ? 11 August 2007 and Spangdahlem a USAFE installation 12-18 Aug 07 1.2. Operational Requirement and Specific Tasks: The Soccer Camp program focuses on -each program will consist of 5 days, 2 sessions of 3 hours per day -Child to staff ratio for the contractor shall be 15 to 1 -Coaches shall be certified by national soccer body, be fluent in the English language and have received training in conducting programs for youth -Each program participant shall receive 15 hours of instruction -Each week the program shall conclude with an awards presentation for the participants and parents -Each base shall receive 2 hours of parents? and/or coaches clinics held 1 evening during the week at no additional charge to the US Government -Coaches shall be trained and certified in First Aid -Base programs shall provide a soccer pitch and goals for the program. -Contractor is responsible for all other equipment and supplies. -The local youth program shall provide lodging and register all program participants. -Local programs shall provide lodging for contractor. -Contractors are responsible for all meals. - Spangdahlem SVYY will provide each individual instructor a Letter Of Identification (LOI) for access to military installation and activities. Contractor shall provide personal data as requested a minimum of 60 calendar days before travel starts. 2.
