- This is combined synopsis/solicitation for commercial items, prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This solicitation is being issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-03 (as of 11Apr 05). The NAICS code 333999, size standard of 500 employees applies to this solicitation. This requirement is 100% set-aside for small Business. This RFQ contains one (1) line items including accessories required. Line Item 0001, CONFINED SPACE RESCUE TRAINING FOR FIRE DEPARTMENT PERSONNEL (1) COURSE OVERVIEW: The Confined Space Rescue course must address rescues involving confined spaces and includes lecture, skills training, and scenario based exercises. The class must cover the use of specialized equipment for atmospheric monitoring, commercial entry and retrieval systems, and employment of rescuer constructed retrieval systems. Participants will gain understanding of confined space hazards and the requirements of the new laws. Special emphasis will be given to rescuer safety, scene management, patient care and movement, and construction and operation of retrieval systems. The confined space rescue course graduate will receive Confined Space Awareness, Operations and Technician certifications (see course descriptions below). The graduate will also be able to receive Pro Board Certification upon completion of the course. The Confined Space Rescue course must meet or exceed the following national and Air Force standards: a) NFPA 1670 Standard - Technical Rescue Training and Operations, b) NFPA 1006 Standard - Professional Competency for Rescue Technician, c) OSHA 29 CFR 1910.146 - Permit Required Confined Spaces for General Industry, d) AFOSH Standards. (2) CONTRACTOR REQUIREMENTS: The training will be conducted for each 24-hour alternating shift. The contractor will provide training for 24 personnel per shift for a total of 48 personnel. The contractor must travel to Misawa Air Base, Japan to provide the training. The contractor will be required to arrange for their own travel and lodging accommodations. The contractor must provide the following: a) Two instructors b) Student manuals c) Certificates of Completion for each Student who completes the class. (3) CLIENT REQUIREMENTS: a) Confined Space/Rope rescue equipment, b) Classroom w/ LED projector, c) Interpreter (if needed), d) Suitable practical training site (4) DOCUMENTATION: Certificates of Completion will be awarded to each Student who successfully completes the course curriculum. All Certificates will be mailed to the Client upon receipt of payment in full. It will be the responsibility of the Client to distribute the Certificates to the Students. (5) COURSE LEVEL DESCRIPTIONS: A) Confined Space Awareness: This 8-hour course is designed to introduce the student to the OSHA 1910.146 standard. Topics include OSHA requirements for confined space entry and rescue, and OSHA position requirements. Additionally, this course introduces the student to atmospheric monitoring and ventilation procedures. Personnel operating at this level will be able to perform certain "non-entry" rescue procedures and manage the scene appropriately. At the completion of the course the Student will understand the OSHA requirement and risk management principles. In addition, students will be able to recognize confined spaces and perform a size up of existing and potential conditions. A focus on non-entry rescue techniques will include identification, assessment and procedures for assisting victims. Process and scene coordination will teach students how to implement the emergency response system for confined space emergencies, and to utilize site control and scene management fundamentals. Specific Topics: -Regulations & Standards, -Permit Entry Program, -Confined Space Hazards, -Atmospheric Hazards & Monitoring, -Ventilation, -Isolation Techniques, and Non-Entry Retrieval. (b) Confined Space Operations: This 16-hour course is designed for personnel who will respond to and perform limited entry and rescue operations or support technician level personnel during confined space entry and rescue operations. At the completion of this course, the student shall learn how to protect him/herself, their team and their victims from the hazards within a confined space. Student will learn how to recognize and size up spaces and manage incidents appropriately, using the guidelines defined by NFPA and OSHA. Areas of focus will include environmental considerations, structural concerns, personal protective gear required and safety. Additional topics include victim transport and procedures for using ropes in a high angle environment. The Operations level course is the first of two rescue entry courses. This course includes topics in confined space rescue rigging, and other operational concerns such as respiratory protection, communications and patient packaging. Students are expected to participate in pipe crawls and perform scenario based rescues. Specific Topics: -Scene Management, -Confined Space Hazards, -Atmospheric Hazards & Monitoring, -Ventilation, -Isolation Techniques, -Retrieval Systems, -Patient Packaging, -Supplied Air Breathing Systems (SABA), -Response Considerations (c) Confined Space Technician: This 16-hour course is the most advanced level of training as outlined in the NFPA 1670 and 1006 standard. This course is designed for organizations and personnel responsible for confined space entry and rescue operations in a more complex environment. Personnel at this level will learn how to manage high risk, high profile confined rescue operations, including management of local, regional and federal rescue teams, size-up and operational strategy for confined space rescue. This is an advanced level course for the serious rescuer, operating at an expert technician level. The Technician level hones the rescuers skills in limited entry and egress rescue. The Technician course is a review of equipment and techniques used learned in the Operations course, with the student applying those techniques to much more complicated environment. Specific Topics: -Scene Management, -Confined Space Hazards,-Atmospheric Hazards & Monitoring, -Ventilation, -Isolation Techniques, -Retrieval Systems, -Patient Packaging, -Supplied Air Breathing Systems (SABA), -Response Considerations. PROVISIONS/CLAUSES. The provision at FAR 52.212-1, Instructions to Offerors -- Commercial Item, applies to this acquisition. The provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items, applies to this solicitation and the offeror must include a completed copy of this provision with their proposal. FAR 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this solicitation. Addenda to FAR 52.212-4: The following additional clauses apply to this solicitation: FAR 52.233-2, Service of Protest: (a) Protest, as defined in section 33.101 of the Federal Acquisition Regulation, that are filed directly with an agency, and copies of any protests that are filed with the General Accounting Office (GAO), shall be served on the Contracting Officer John Rogers (addressed as follows) by obtaining written and dated acknowledgement of receipt from 35th Contracting Squadron, Attn: TSgt John Rogers, Unit 5201, APO AP 9639-5201.
