Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is W9114F-07-R-0031. This solicitation is issued as a Request For Proposal, which incorporates provisions and clauses for commercial items in effect through Feder al Acquisition Circular 2005-16. This is an unrestricted acquisition. The North American Industry Classification System (NAICS) code is 334517. Item 0001 EXCIMER REFRACTIVE SURGERY LASER, Laser will include all equipment needed to perform conventional and Wavefront guided refractive surgery treatments, gas bottles for 1,000 treatments, licensing fee for 1,000 treatments, installation, training ( in English), trade-in and removal of the current refractive surgery laser, and 1 year warranty. Equipment must be FDA (U.S. Food and Drug Administration) approved. Item 0002 CONTRACT MANPOWER REPORT Required voltage for all electrical equipment is 220V/50Hz Delivery of equipment is to LANDSTUHL REGIONAL MEDICAL CENTER PROPERTY MANAGEMENT BR GEB 3739 LANSTUHL/KIRCHBERG 66849, GERMANY Required delivery: 30 days after receipt of order.
