- Sources Sought Announcement This is a sources sought announcement only.   This is not a solicitation announcement.     The purpose of this announcement is to gain knowledge of potential qualified sources such as Service-disabled veteran-owned small firms, Veteran-owned firms, Hubzone firms, general small business firms and any other types or size of business relative to NAICS 621498, size standard $9 million capable of providing the following services. The Network 22 Logistics Office (VISN22) is seeking sources for establishing a Community Based Outpatient Clinic (CBOC) in the Imperial Valley, CA area. The Department of Veterans Affairs Healthcare System in San Diego (VASDHS) has a requirement for Community Based Outpatient Clinic (CBOC) services in El Centro, CA or within a five mile radius of El Centro, CA. The contractor is to provide primary care and telemental health services on a full service - all inclusive capitated rate basis. The annual capitated rate per patient shall represent a Firm-Fixed Price, Indefinite Delivery, and Indefinite Quantity (IDIQ) all-inclusive reimbursement rate for the performance of all services. The contractor facility must provide services exclusively to veteran patients only. Interested firms having the skills and capabilities necessary to perform the stated requirements are invited to provide information to contribute to the market survey/sources sought notice. Responses to this Notice must provide the following information: 1. General Information of the company name, address, phone number, fax number, e-mail address(s), and company web site. 2. How long has your company been in business. 3. What services does your company primarily provide and for how long. 4. Does your company have experience with the Department of Veterans Affairs capitated CBOC program? 5. What relevant Government and commercial experience does your company have related to establishment of CBOCs? 6. Describe how you intend to provide the Capitated CBOC services for our Veterans. 7. Describe your company ownership composition listing the names of individuals that control and have authority to enter into a contract with the Government and their percentage of ownership of your company. 8. Will your company be establishing a joint venture with another company? If so, list the names and provide the company's general information, their responsibilities, and authority of managing of the contract. 9. Does your company meet the standards of Joint Commission Accreditation of Hospital Organizations (JCAHO)? 10. Provide the type (i.e. small, veteran owned) and size of your business (and those of any intended joint ventures/partnerships) relative to NAICS 621498, size standard $9 million. 11. Provide your Dun and Bradstreet Number and Tax I.D. Number.
