Description

DESCRIPTION/SPECIFICATION/WORK STATEMENT Contractor shall provide 24/7 NeuroRehabilitation Service to include: 24 hour supervision exclusive of intensive 1;1 care required for behavioral management. Room and board. Client trust fund service for client's personal funds Personal laundry services Evaluations ,treatments and services provided by contractors clinicians indlucing neuropsychology,behavorial specialist,speech/language pathology,occupational therapy,physical therapy, educational programs,nutritional,cognitive therapy,recreational services and activities, avocational/prevocational, nursing& health Program transportation. NOT INCLUDED IN THIS CONTRACT ARE AS FOLLOWS Intensive 1;1 supervision Diagnostic or routine lab services,diagnostic or routine x-ray or testing. Pharmaceutical services, medications(prescriptive or OTC.) Physician services including but not limited to-annual exams by specialists,diagnostic,emergency, routine care consultation or ongoing treatment services whether performed on-site or off-site at a physicians office, outpatient services, inpatient hospital care, or any other location care may be performed. Transportation to out of area appointments. Ambulance service, ground or air. DME rentals or purchases Adaptive or facilitative equipment and supplies. Orthotic and prosthetic services.
