Description
PRE-SOLICITATION NOTICE: The VA National Acquisition Center will issue Request for Proposal (RFP) VA-797-07-RP-0077 for the Department of Veterans Affairs (VA), and Department of Defense (DoD) to procure Lorazepam Tablets. The estimated annual usage is as follows: 45,638 bottles of Lorazepam Tablets 0.5MG, 100s; 31,168 bottles of Lorazepam Tablets 0.5MG, 500s; 77,747 bottles of Lorazepam Tablets 1MG, 100s; 46,106 bottles of Lorazepam Tablets 1MG, 500s; or 23,053 bottles of Lorazepam Tablets 1MG, 1000s; 13,808 bottles of Lorazepam Tablets 2MG, 100s; 8,904 bottles of Lorazepam Tablets 2MG, 500s. Where there is a choice of package size as indicated by the word "OR", the offeror must submit a price on at least one of the two sizes indicated. Award will be made on the package size most advantageous to the Government and included in the aggregate (all or none) award based on the lowest total evaluated price of all line items offered, taking into consideration the estimated quantities. The estimated annual usage listed above is for VA, and DoD, Indian Health Service (IHS) and Bureau of Prisons (BOP). VA reserves the right to add additional package sizes to the contract at a later date, when they become available or if offered in a vendor's proposal. This will not affect the quantities listed in the usage requirements. The contract period will be for one year plus four one-year option periods. The contract items will be distributed through the VA and DoD's respective Pharmaceutical Prime Vendor(s). Offeror's must state the exact name (name that appears on label) by which the drug will be supplied and have a unique NDC number. Tablets must be compatible with automated dispensing units (Baxter ATC Canisters, Optifil, etc.). If the offeror is not a manufacturer, a signed agreement between the offeror and the manufacturer must be submitted with the offer and remain in effect for the duration of the contract period including the option years. Solicitation No VA-797-07-RP-0077 will be electronically issued in commercial item format in accordance with FAR Part 12 on or about July 27, 2007 with a closing date of August 17, 2007. The NAICS code for this procurement is 325412. Any future amendments and other miscellaneous documents will be available electronically for download at the following URL: http://www.fedbizopps.gov. No paper copies of the solicitation will be available, and no telephone requests for paper copies of the solicitation will be accepted.
