Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the Federal Acquisition Regulation (FAR) subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only sol icitation; quotes are being requested and a written solicitation will not be issued. No telephone requests will be accepted. The solicitation number for this Request for Quotation is N6847007062004. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-17. This solicitation is full and open competition; the associated NAICS code is 334510. This requirement is for the U. S. Naval Hospital, Okinawa, Japan. ITEM DESCRIPTION: 0001 BIS (BISPECTRAL INDEX) VISTA MONITOR 100-240 VAC, 50-60 Hz, conforms to : UL 60601-1, IEC 60601-2-26, CAN/CSA-C22.2 #601.1, USB PORTS A,B,RS232 SERIAL PORTS. Cat. Number 186-0215 (Quantity 5 EA) OR EQUAL; 0002 XP BIS QUATRI SENSOR, Cat Number 186.0106 (Quantity 200 EA). The monitoring system monitors brain activity and level of consciousness/awareness during general anesthesia. Delivery, which shall be to U. S. Naval Hospital, Okinawa, Japan, shall occur within 30 days after award of purchase order. FOB Destination pricing is requested. Offerors responding to this announcement shall provide all information contained in the Federal Acquisition Regulation (FAR) Provision 52.212-1, Instructions to Offeror - Commercial Items. As stated in this provision the Government intends to evaluate offers and award a contract without discussions with offerors. Therefore, the offeror?s initial offer should contain the offeror?s best terms from a price and technical standpoint. However, the Government reserves the right to conduct discussions if later determined by the Contracting Officer to be necessary. 52.212-2, Evaluation -Commercial Items. The following factors shall be used to evaluate offers: a) Technical b) Past Performance and c) Price. If offering an equal item, provide comple te technical documentation and specifications to be able to evaluate. 52.212-3, Offeror Representations and Certifications-Commercial Items; an authorized representative of the offeror must sign these representations and certifications and must be included with the offer. The resulting firm fixed price award will incorporate the requirements of the following clauses: 52.212-4, Contract Terms and Conditions - Commercial Items; 52.212-5, Contract Terms and Conditions Required to Implement Statutes Or Executive Orders - Commercial Items, with applicable FAR clause: 52.222-3; 52.222-21; 52.222-26; 52.232-33; Quotes are due by close of business June 29, 2007, JST ( Japan Standard Time) to the address shown in this synopsis.
