Description
This is a combined synopsis/solicitation. Request for Quotation #RFQ VA-69D-07-RQ-0208 entitled "Endoscopy Technician Services" is hereby issued for commercial items as Full and Open Competition and is prepared in accordance with the format in Federal Acquisition Regulation (FAR) Part 12, Acquisition of Commercial Items, Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, Part 13, Simplified Acquisition Procedures, and as supplemented with additional information included in this notice. This announcement incorporates provisions and clauses in effect through FAR FAC 2005-16 and VAAR 97-10. NAICS Code 621999 applies for this procurement and the small business size standard is $9.0 million. This announcement/solicitation constitutes the only bid document that will be issued. A separate written solicitation document will not be issued. Contractor's proposals (cost and technical) are herein requested. A response to requirements listed below shall be submitted on contractor's letterhead or formal quotation form and must address all requirements listed in a separate technical proposal. Contractor's price and technical proposal must contain the name; title and signature of person authorized to submit proposals/bids on behalf of the contractor. Jesse Brown VA Medical Center, 820 S. Damen Avenue, Chicago, IL has determined a requirement for TWO (2) Endoscopy Technicians to work eight (8) hours per day, Monday through Friday for the period of July 1, 2007 through September 30, 2007. Contractor to provide services of up to two (2) qualified and experienced Endoscopy Technicians. The Endoscopy Technician functions as an integral part of the team that provides care to adults with gastrointestinal and/or pulmonary disorders/diseases. He/She is accountable to and performs duties under the direction of an RN when providing delegated patient care activities after demonstrating competency, within the following limits: assists in collecting data for an objective assessment to identify the patient's needs, problems, concerns or human responses, e.g. vital signs; assists under the direction of a RN or physician in the implementation of established plan of care; assists the physician and RN during diagnostic and therapeutic procedures. Employees must be able to appropriately attach a patient to an EKG machine and perform a twelve lead EKG rhythm strip. Retrieve old EKG's using the MAC VU System. Properly hooks patient to cardiac, blood pressure and 02 saturation bedside monitoring machine. Performs pulse oximetry. Must be able to quickly identify a patient in acute distress and rapidly notify the appropriate licensed practitioner. Is proficient with electronic documentation using the GI software. The role of the Endoscopy Technician in the endoscopy setting, while keeping the Nurse Manager abreast, also includes: keeping endoscope inventory; demonstrating knowledge and competency in disinfecting, manual washing of different endoscopes; performing Leak Test and inspecting endoscopes for any visible wear and tear; doing biological and chemical monitoring and keeping an accurate record of results; identifying, sending and keeping record of broken endoscopes and disinfection equipment or automatic reprocessor for repair; cleaning and packaging any instrument/device for reprocessing. The Endoscopy Technician demonstrates knowledge and maintains principles of infection control in his/her practice. Tech must have experience in assisting GI and surgical procedures, such as colonoscopy, EDG, ERCP and sigmoidoscopy. Tech must have a valid CPR card. The contractor must provide the following documents for each contractor employee working under this contract: qualifications for the job; a current competence assessment checklist (an assessment of knowledge, skills, abilities and behaviors required to perform this job correctly and skillfully; a current performance evaluation supporting ability of the contractor employee to successfully perform the work required and a listing of relevant education), current CPR card, certificate of liability insurance. A full-time equivalent (FTE) is defined as a contractor's employee working eight (8) hours per day, five (5) days per week (40 hours per week). Special Agreement Check (SAC) - FBI criminal history check of fingerprints is required prior to contract performance. Contractor's employees must be present at VA and must be actually performing the required services for the period specified in the contract or the contract cost will be decreased accordingly during each billing cycle. A VA record-keeping system of contractor hours worked is established through time and attendance logs in the departments being served. Payments to contract personnel for any leave, including sick leave, are the responsibility of the contractor. VA is not obligated to the Contractor during technicians' absence. Evaluation of proposals offered will include the following factors: Professional qualifications, immediate availability, past performance and price. Multiple awards may be made. Award will be made to the contractor(s) determined to be the "best value" to the Government. Evaluation may be made with or without discussions with the offeror(s). All responsible sources that can meet the requirements and provide services as listed above may respond to this solicitation by submitting cost and technical proposals (curriculum vitae) on their company letterhead or bid form.
