- ATTENTION POTENTIAL OFFERORS: DUE TO ESTABLISHED ECONOMIC AGREEMENTS BETWEEN THE UNITED STATES GOVERNMENT AND THE GOVERNMENT OF TURKEY, THE AIR FORCE INTENDS TO AWARD THIS SOLICITATION TO A BUSINESS CONCERN WITHIN THE COUNTRY OF TURKEY. THEREFORE, PROPOSALS FROM BUSINESS CONCERNS OUTSIDE OF TURKEY WILL NOT BE CONSIDERED (i) This is combined synopsis/solicitation number F2G3D87135A200 issued on 18 June, 2007 for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Solicitation F2G3D87135A200 is issued as a Request for Quote. (iii) The solicitation document and FAR incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-17 and Class Deviation 2005-o0001 (iv) This acquisition is being changed from 100% Small Business Set-Aside to Unrestricted due to the Overseas Location Exception. (v) The 39.th Contracting Squadron has a requirement for: (1) AEROBICS AND PILATES INSTRUCTOR AT THE INCIRLIK AIR BASE FITNESS CENTER, Quantity: 360; Unit of Issue: Sessions. (vi) Detailed Description: PROVIDE AEROBICS AND PILATES INSTRUCTOR AT THE INCIRLIK AIR BASE FITNESS CENTER. ONE INSTRUCTOR IS REQUIRED PER CLASS FOR EACH SCHEDULED CLASS. INSTRUCTOR MUST BE CERTIFIED TO TEACH BOTH AEROBICS AND PILATES THROUGH A NATIONAL FEDERATION SUCH AS -- (AFAA) AEROBICS & FITNESS ASSOCIATION OF AMERICA, (AAAI) AMERICAN AEROBICS ASSOCIATION INTERNATIONAL, (ACSM) AMERICAN COLLEGE OF SPORTS MEDICINE, (ACE) AMERCIAN COUNCIL OF EXERCISE AND (CIAR) COOPERS' INSTITUTE FOR AEROBICS RESEARCH. CLASSES WILL NOT BE HELD DURING FEDERAL HOLIDAYS/THREE DAY WEEKENDS, WING GOAL- GOAL/DOWN DAYS & EXERCISES. THIS CONTRACT WILL HAVE 4 OPTION YEARS. OPTION YEAR 1: 1 APRIL 2008 ? 31 MARCH 2009, OPTION YEAR 2: 1 APRIL 2009 ? 31 MARCH 2010, OPTION YEAR 3: 1 APRIL 2010 ? 31 MARCH 2011, OPTION YEAR 4: 1 APRIL 2011 ? 31 MARCH 2012. (vii) Period of Performance: 1 October 2007 ? 31 March 2008. (a) The Government will award a purchase order resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government using the factors from 52.212-2. FITNESS INSTRUCTOR PERFORMANCE WORK STATEMENT QUALIFICATIONS: Contractor and any employees shall be certified by a national or internationally recognized organization. All other qualifications must be by an approved Aerobic and Pilate?s institution and easily identified. Formal certified institutions are the American Aerobic Association International (AAAI), International Sports Medicine Association (ISMA), International Aerobic Federation (IAF), Step Reebok, YMCA, RSA and Adjustable Super Step #1. CANCELLATION FEE: The contractor shall be responsible for paying in full a cancellation fee to cover the cost of a substitute contractor if at least (24) hour?s notification for non-attendance is not received. Activity shall be responsible for paying contractor for the cost of a class if at least (8) hour?s notification is not received for a cancelled class. Neither the activity nor the contractor shall provide a substitute for any class without prior approval by the contracting officer. The contractor shall provide and be responsible for the following: 1. Provide mandatory fitness improvement for Incirlik Air Base through the Fitness Center. 2. Coordinate with Exercise Physiologist concerning expectations of the class. 3. Provide current proof of insurance for individual or individuals performing the service prior to award of contract. Should at any time the contracting office and/or Fitness Center Director or NCOIC Sports & Fitness request to see the current proof of insurance; the contractor must provide all such documents for the contract period with in 24 hours. 4. Provide proof of current CPR certification 5. Maintain a current list of certified employed with the contracting officer. 6. Provide the Fitness Center Staff with an organized schedule of projected class events monthly. 7. Ensure qualified instructors that have met the same requirements as listed in numbers 3 and 4 and have documentation on file with contracting officer are available for all scheduled classes. 8. Perform instruction for 60 classes per month 7 days a week according to schedule set by Fitness Center Director and/or NCOIC Sports & Fitness. Class schedule may be adjusted on a monthly basis based on class headcounts and needs of the customers. 9. Provide exercise music and necessary equipment required for each scheduled class. 10. Notify the Fitness Center Director and/or NCOIC Sports & Fitness at least (24) hours prior to class date and time if unavailable for a said class, providing reason for non-availability. 11. Report to the Fitness Center Director and/or NCOIC Sports & Fitness or Fitness Staff at least 15 minutes prior to class commencement, wearing appropriate workout attire in order to promote a neat, clean, and professional image. 12. Any and all questions or concerns relating to schedules, classes, cancellations will only be discussed between the Contracting Officer, the contractor and Fitness Center Director and/or NCOIC Sports & Fitness. 13. Provide an overview of the workout plan to include safety briefing, warm-up and stretch exercises, and prior to commencing aerobic exercise. Provide instructions on achieving maximum heart rate, targeted heart rate and targeted heart rate training zone and how to monitor the pulse before, during and after the exercise. The Director and/or NCOIC Sports & Fitness must approve workout plan. 14. Incorporate different workout levels in the program and ensure a minimum of 20 minutes training is in the targeted heart rate training zone is accomplished, in addition to floor workout to tone abdominal, upper and lower extremities. 15. Ensure class size does not exceed a maximum of 30 participants. 16. Classes will be taught on a 48 week yearly schedule and will not exceed 60 classes of instruction per month. Classes will not be held on Federal Holidays, Wing events, Family and Goal Days, Three-day Weekends and the last Friday of each month from 0800-1200 HRS during unit training. In addition classes will not be held from 20 Dec ? 2 Jan.
