- THe Centers for Medicare & Medicaid Services (CMS) intends to award a sole source follow-on contract to Iowa Foundation for Medical Care (IFMC), 6000 Westown Parkway, Des Moines, Iowa, 50266. This contract will continue the Diabetes Self Management Testing (DSMT) validation process. The oversight validation process will assist CMS in its effort to determine whether approved DSMT national accreditation organizations are functioning in the manner as required under Medicare regulations. IFMC will perform the oversight validation process by completing evaluations of the American Diabetes Association (ADA) and the Indian Health Services (IHS) accrediation programs, which are national DSMT acccrediation organizations approved by CMS for deeming authority. The IFMC will utilize the protocol and instruments developed under Phase I in determining whether (1) the national accreditation organization effectively applies and enforces the CMS quality standard described in &410.144 of the Naitonal Standards for Diabetes Self Management Education Programs (NSDSMEP);(2)the desk audit and on-site review findings obtained by IFMC are consistent with the ADA and IHS findings of the same DSMT entities; and (3) the accreditation organization adheres to their established policies and procedures. The following services are classified under NAICS Code 541690. The period of performance will be a base year and four (4) one year options. All interested parties shall be requiraed to provide a capability statement. The capability statement must not exceed 10 pages and can be emailed to joanne.day@cms.hhs.gov. All capability statements will be evaluated by the program office. No fax copies of the capability statement will be accepted.
