- Woodland Job Corps Center On-site Medical Services We are currently soliciting written quotes for the following on-site medical services: 1. Center Physician. The schedule is 12 hours per week, Tuesdays and Thursdays 3:00pm ? 9:00pm. There is a small window of flexibility with the hours. Please reflect any requested hours that vary from the above published hours in your quote. Written quotes should include cost price per hour per scope-of-work for the following time period (please call for copy of scope): 1 Year, July 16, 2007 to April 30, 2008. 2. Center Dental Hygienist. The schedule is 12 hours per week, Tuesdays and Thursdays 1:00pm ? 5:00pm. There is a small window of flexibility with the hours. Please reflect any requested hours that vary from the above published hours in your quote. Written quotes should include cost price per hour per scope-of-work for the following time period (please call for copy of scope): 1 Year, July 16, 2007 to April 30, 2008. 3. Center Dentist. The schedule is 12 hours per week, Tuesdays and Thursdays 1:00pm ? 5:00pm. There is a small window of flexibility with the hours. Center Mental Health Consultant. The schedule is 9 hours per week, Thursdays 1:00pm ? 10:00pm. There is a small window of flexibility with the hours. Please reflect any requested hours that vary from the above published hours in your quote. Written quotes should include cost price per hour per scope-of-work for the following time period (please call for copy of scope): 1 Year, July 16, 2007 to April 30, 2008. Renewals will be awarded at the discretion of the Center. All written quotes should reference Solicitation Number and be submitted to Donald Hickman, Purchasing Agent, no later than 4pm EST on Friday July 6, 2007. Quotes may be submitted electronically to hickman.donald@jobcorps.org or via fax at 301-498-7044. The Center reserves the right to reject any bid and select the bid deemed most responsive.
