- Description
- The contractor shall provide all necessary supplies, materials, equipment, and labor to perform transcription of medical documents, primarily Radiology Terminology, at the contractor facility with contractors furnish equipment for the VA Medical Center, 800 Zorn Avenue, Louisville, KY 40206 and Ireland Army Community Hospital, FT. Knox, Kentucky (IACH) for the period of October 1, 2007 through September 30, 2008 with four (4) one-year options to renew. Contractor's facilities must be located in the United States. THIS IS A TOTAL SMALL DISABLED VETERAN OWNED SMALL BUSINESS SET ASIDE. The procurement is under Part 12 of the FAR Commercial Items. The following clauses and addendum apply: 52.212-1 Instruction to Offerors, 52.212-2 Evaluation, 52.212-3 Representations and Certifications, 52.212-4 Terms and Conditions and 52.212-5 Terms and Conditions to Implement Statutes or Executive Orders.
