- This is a notice that the Department of Veterans Affairs intends to negotiate, under the authority of 41 U.S.C. 253 (c) (1), a sole source purchase order with Sierra Scientific Instruments, 5757 Century Blvd, Suite 660, LOS ANGELES, CA 90045. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice in conjunction with procedures in FAR Part 13. This announcement constitutes the only solicitation, and a written solicitation will not be issued. VAPAHCS, Palo Alto, CA has a requirement to procure the following items: One (1) each ManoScan 360 Esophageal Motility System, Includes the following: 36 element high-resolution esophageal catheter, ManoScan data acquisition and ManoView analysis software, Data Controller Unit, System Cart with built-in calibration chamber and isolation transformer, 17" LCD Touch Screen Monitor, 52x speed CD read/write, 1200 dpi color inkjet printer, 256 Megabyte portable memory card,("data key") keyboard, mouse with mouse pad, wrist guard. One year of free software upgrades, On-site system installation, UL2601-1 Compliant Includes a box of 10 disposable catheter sheaths; and One (1) each High-Resolution Esophageal Catheter, 36 channel circumferential solid-state catheter probe. Delivery location is VA Medical Center, Warehouse, 3801 Miranda Avenue, Palo Alto, California 94304-1290. This notice is not a request for competitive proposals or a solicitation for offers. The NAICS code is 334510 and the size standards is 500 employees. Provisions and Clauses are those in effect through Federal Acquisition Circular 2005-17. The follow Solicitation Provisions/Contract Clauses are applicable: FAR 52.212-4 Contract Terms and Conditions-Commercial Items (Sep 2005), FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items (Sept 2006). For the purpose of this clause items (b) 5, 16, 17, 18, 19, 20, 24, and 34 are considered checked and apply, and VAAR 852.270-4 Commercial Advertising (Nov 1984).
