Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is W91YTV-07-T-0169. This acquisition is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effe ct through Federal Acquisition Circular 2005-12. The North American Industry Classification System Code (NAICS) for this procurement is 423450. This requirement is 100% total Small Business Set a-side. The Blanchfield Army Community Hospital, Fort Campbell, Kentucky, has a requirement for the following laser equipment: CLIN 0001: 100 V Laser, Weight and Size Requirements: Weight  88lbs, Dimensions  26 inches by 18 inches by 16 inches. Cart Dimensions  22 inches by 22 inches by 30 inches. A s ervice program guaranteeing overnight shipping of a replace system, if necessary, as long as the unit is under warranty or service contract. Platform technology ability to add future modules to platform purchased. Specifically new technology that becomes available in the future. System that houses both laser and light based systems. A complete laser system that has the following laser modules/wavelengths: 2940 erbium module, 2940 pixel (fractionated) module, q-switched 532/1064 module, 1064 lp/yag, 132 0 module, 320/uvb module, 780-950 skin tightening module, advanced fluorescence technology light based hand pieces with the following wavelengths: 515, 540, 570, 650, and 420. 1 Each $________ . Please include shipping cost and date of shipment. The G overnment anticipates awarding a single award for this requirement. The items shall be delivered to the Blanchfield Army Community Hospital, Property Management Branch, Bldg 650, Level 1 Joel Drive, Fort Campbell, KY 42223-1498 within 30 days after recei pt of order.
