- Description: This is a solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. This solicitation is set aside for local business located within the effected areas of Hurricane Katrina within the state of Alabama in accordance with the Robert T. Stafford Disaster Relief and Emergency Assistance Act. Solicitation Number HSFESA-07-Q-0001 is being issued as a Request for Quotes (RFQ), and the Federal Emergency Management Agency intends to award one purchase order to meet the stated requirements. Specifically, the Federal Emergency Management Agency (FEMA) has a requirement for the following line item(s): Line Item 0001, Rental of four (Qty 4) 5,000 Lb capacity propane powered (Cushion tire) forklifts for a period of six (6) months from the date of award. Each forklift shall be provided with 2 extra propane tanks. Forklifts should be complete with all OSHA required safety features. Costs associated with maintenance shall be included in quotes. The forklifts will be delivered to the following location: 861 Plantation Way, Montgomery, AL 36117. Delivery and pickup costs shall be included in quotes. The following provisions apply to this solicitation: 52.212-1 Instructions to Offerors ? Commercial Items 52.212-2 Evaluation ? Commercial Items 52.212-3 Offeror Representations and Certifications ? Commercial Items. The vendor must be registered with ORCA. Vendors may register electronically at http://orca.bpn.gov/. The resulting purchase order will include the following clauses: 52.212-4 Contract Terms and Conditions 52.212-5 Contract Terms and Conditions Required to Implement Statures or Executive Orders ? commercial Items. 52.233-4 Applicable Law for Breach of Contract Claim 52.232-25 Prompt Payment 52.232-33 Payment by Electronic Fund Transfer ? Central Contractor Registration. The vendor must be registered with CCR. Vendors may register electronically at http://www.ccr.gov/. HSAR 3052.209-70 Prohibition on Contracts with Corporate Expatriates. Basis for Award: If offers are received from two or more small business concerns located within the effected areas of Hurricane Katrina within the state of Alabama then this procurement will be set aside for small businesses and proceed accordingly. Otherwise, the procurement will proceed as a set aside for businesses located within the effected areas of Hurricane Katrina within the state of Alabama. Interested vendors must provide the address of their business location and business size for verification. If no offers are received from businesses located within the areas affected by Hurricane Katrina within the State of Alabama then this solicitation will be canceled in its entirety.
