Description

DME HME Consolidated for four Michigan Sites Provide repair services, labor, materials, patient education and equipment for Home Medical Equipment (HME) and Durable Medical Equipment (DME) for VA facilities located in Ann Arbor, Detroit, Battle Creek, and Saginaw, Michigan. Contractor shall meet all requirements of the contract. NAICS 532291. Projected period of service is from October 1, 2007, through, September 30, 2008 with four pre-priced optional renewal years. Requests for proposals are now available. Requests may be obtained directly from this website or by written request to the Contract Officer. Requests may also be requested via facsimile at 734-213-3833. Contractors are responsible for verifying that requests have been received in the contracting office. Telephone or internet requests will not be accepted. All request must reference VA 251-P-0004. Proposals are due in the contract office by Friday, August 3, 2007 at 3:00 pm local time, in Building 3, Rm R02 if handcarried to the VA Healthcare System, 2215 Fuller Road, Ann Arbor MI 48105 (mail code 54C). Proposal received after due date will not be accepted.
