REVISED SCHEDULE FOR VA 251-P-0004 II. SCHEDULE OF SUPPLIES/SERVICES WORKLOAD REQUIREMENTS Durable Medical Equipment (DME) / Home Medical Equipment (HME) SERVICE - Furnish all labor, materials, patient education and equipment necessary to transport Home Medical Equipment (HME) /Durable Medical Equipment (DME) and accessories and provide maintenance (repair, preventative, and cleaning) of HME and DME and accessories within the Department of Veterans Affairs Beneficiaries residences, throughout lower Michigan and parts of Ohio, for a period beginning from award of contract through September 30, 2008, with four (4) renewal option years through September 30, 2012. An offer will not be considered for award if the offer does not include all required pricing information (i.e., unit price and estimated total cost for all items that require input of information, etc.) for the base period and all four option years under the individual location. BASE YEAR Reference 4.9.5 (a)-(d) when providing proposed pricing. SCHEDULE OF SERVICES TRANSPORTATION OF MEDICAL EQUIPMENT AND/OR ACCESSORIES TO CLOSEST DELIVERY SITE (i.e. areas identified in contract to cover Ann Arbor, Battle Creek, Detroit, Saginaw VAMC or Toledo, OH). VA will not pay additional mileage if contractor fails to work from storage site/inventory location other than the one closest to patient and nearest VA site. Storage sites should be identified in proposal and ideally within a 20 mile radius from each VA facility. Period of service will be from date of award through September 30, 2008. ESTIMATED PRICE PER TOTAL QUANTITIES UNIT UNIT PRICE 1. Standard delivery /or/ recovery of items only (no special instructions or set up) such as wheelchairs unpackaged or small packaged items. Recovery 540 trips ______ ________ of Telehealth devices.* 2. Standard delivery /or/ recovery of items requiring set up and instructions e.g. hospital beds, trapeze, tub transfer, wheelchair, overbed table, etc., when delivered as a unit to the patient. 150 trips ______ ________ 3. Home dialysis set up, complete, including hookup, test and verification the equipment is working. 4 trips ______ ________ 4. Pick up/exchange of dialysis and/or reverse osmosis machines, including trouble shooting, hookup and test. 72 trips ______ ________ 5.
