- This is a combined/synopsis solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation reference number is F3Z4537191A003; this solicitation is a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-18 and Defense DCN 20070531. The associated Standard Classification (SIC) is 8621. The North American Industry Classification System is 611430 and the business size standard is $6,000,000.00. The Federal Supply Class (FSC) is R499. This solicitation is 100% small business set aside. The government intends to solicit and award this purchase order for the following item: CLIN 0001: Services, nonpersonal: Contractor to provide all labor, materials, parts, transportation and other necessary item to provide mock accreditation to include comprehensive review of the organization utilizing the tracer methodology. Contractor will utilize the Joint Commission Agenda to increase awareness of the Staff as to what to expect during an actual survey. Services shall be performed in accordance with the attached statement of work. Performance period will be 25 through 28 Sep 2007. Place of performance is Bldg 777, David Grant Medical Center Travis AFB, CA 94535. The following provisions or clauses apply to this acquisition. Mock Accreditation Survey Services, nonpersonal: Contractor to furnish all labor, tools, parts, materials, transportation, and all necessary item to provide an on-site ?Mock Accreditation? survey to provide a comprehensive review of the organization utilizing the tracer methodology. Contractor will utilize the Joint of Commission Agenda to increase awareness of the staff as to what to expect during an actual survey. The objectives are: to identify areas of non-compliance, to provide education to staff, and to develop corrective action plan. Contractor shall provide three people for four days along with one LSC Specialist for two days three days of tracing followed by one day of education to integrate the findings for the tracers and the life safety code visit. STATEMENT OF WORK The 60th Medical Group requires the services of an external source to perform an on-site mock accreditation survey and conduct an education program based on the Joint Commission (TJc) standards. TJC is the Joint Commission is the standards-setting and accrediting body in national health care, and is utilized by the DoD for inspection and accreditation of its large healthcare centers. It is desired that these events occur in tandem from a single source for the sake of staff interaction, continuity, and confidence in the external source. The mock survey shall be performed by a skilled contractor-clinician (physician, nurse, behavioral health professionals, etc), who possesses deep knowledge of accreditation standards for hospitals, ambulatory centers, and behavioral health organizations. It is requested that the external source possess deep experience in the techniques employed by Joint Commission Surveyor in conducting actual organization surveys under the auspices of TJC. The 60th Medical Group requests a three-day mock survey, along with a one-day education program for 60 MDG staff. Capacity for the program is requested to be up to 200 attendees, and external source shall provide education materials that address TJC accreditation standards and patients care functions. At the conclusion of the engagement, a mock survey report will be provided to the Commander, 60th Medical Group. This report will comprehensively address areas identified by the mock survey that require attention. The report should aggregate and score the findings of the survey, and accurately represent that state of preparedness of the 60th Medical Group. Location of Work: The mock survey will be conducted within the facilities of the 60MDG, David Grant Medical Center Travis Air Force Base CA 94535. Period of Performance: Service shall be provided from 25 through 28 Sep 2007. Final report shall be provided to the Commander, 60th Medical Group, 101 Bodin Circle, Travis AFB CA NLT 15 October 2007.
