Description
00HCTBBG-2007-43627 The Centers for Disease Control and Prevention (CDC), National Centers for Environmental Health (NCEH) intends to award, on a sole source basis, a purchase order to the Alliance For Healthy Homes (AFHH), 1825 K Street NW, Suite1100, Washington, DC 20013-0941, as it is believed that the AFHH possesses unique qualifications and demonstrates a unique capability to provide the following environmental consulting services: Scope of Work: The contractor will develop 1) an inventory of non-HUD housing subsidies or low income tax credits; and 2) draft educational memoranda for health and housing agency staff and the general public on lead(Pb)-safe housing standards and strategies to assure compliance with those standards. The inventory will be a principle resource for the materials. Period of Performance: 12 months from effective date of award of a U.S. Government Purchase Order. Deliverables: 1. Develop an inventory of non-HUD housing subsidies or low income tax credits. This inventory will be used in the deliverables below. 2. Develop simple advisory bulletins that inform multiple audiences of the need and opportunity for expanding access to lead-safe housing. The draft bulletins must be in a format for CDC clearance and public release. There are to be 4 to 5 bulletins, not to exceed 10 pages, identified by target audience. Updated Project Plan: Shall include identification of materials to be developed, brief summary of how they are to be developed, pilot testing, time frames, identification of CDC or other resources necessary for project completion, and recommended format of the final report including a draft outline of expected content. The project plan should be based on the technical proposal. 4. Interim Progress reports - Summary progress towards completing each element of the project, including any changes in the project plan and copies of materials competed to date and materials submitted to CDC for comment. Changes in the project plan must be approved in advance by the CDC. 5. Final Report, tentatively titled: Recommendations, Resources, and Training for Expanding Access to Lead-Safe Housing. The report will include all materials developed for the project, a brief summary of the materials developed and/or collected including their intended audience and recommended method of delivery, and an index of related materials in support of the material content (references, other related resources, where to go for further information). These materials shall be delivered as one organized, combined unit even if portions have been previously provided to the CDC. It shall be provided electronically, with two hard copies suitable for reproduction. The contractor may post these materials on its website, pending final clearance by the CDC or determination that CDC clearance is not required. Training materials, which take the form of classroom and distance learning modules for use in relevant training centers and other structured group settings, consist of annotated slides supplemented by handouts; guidance materials are concise reading materials containing the same core information as the training materials but better suited to individual downloading and study. When appropriate, the simpler and less expensive training method should be selected. For example, interactive training is not expected. The method and format of the training materials shall be approved by the CDC upon review of the project plan prior to implementation. Limited pilot testing of the training materials shall be conducted. The contractor shall work with CDC program and clearance staff to provide edits and revisions of materials for release by the CDC during the development phase and meet requirements for the documents to be cleared by the CDC for public release. (See document: NCEH/ATSDR Policy Clearance of Information Products 3/06.) Allow at least 8 weeks for the clearance process, not counting time spent providing requested edits or revisions. All graphics, photos, images, language, etc. shall be CDC property, public domain, or otherwise allowable for CDC release in a public, copy able document. Cleared documents shall be reviewed with CDC MASO for acceptability for printing prior to accepting final delivery. Any changes required by MASO shall be made before acceptance of final delivery. ? Electronic pre-press files furnished for printing must be created using Mac OS System 10 or later or Microsoft Windows w/2000 or XP on a single session Recordable CD or DVD, using the software Adobe CS2 or QuarkXpress 6.1. Contractor must also furnish a press optimized PDF from the finalized print files. Note: Contractor must use software in corrected native format, latest version, and without third party extensions/plugins. Also, media must include all fonts used. In addition to CD/DVD, contractor must furnish laser print (in color as required) hard copy of final file. The contractor will provide an electronic version of each final document suitable for posting on CDC?s website that is ?508 compliant?. See CDC guidelines for 508 compliance. ? Section 508 (29 U.S.C. ? 794d) requires that Federal agencies' electronic and information technology is accessible to people with disabilities. Under Section 508, agencies must give disabled employees and members of the public access to this technology that is comparable to the access available to others. The law applies to all Federal agencies when they develop, procure, maintain, or use electronic and information technology. All materials and their content will be the property of the CDC. All reports and deliverables shall be made to CDC PGO for official record keeping, with a courtesy copy to the CDC Project officer. All reports will be in electronic format in MS Word, Excel, and PowerPoint formats, with two hard copies each of the Final Report to the CDC Project Officer. All materials provided will be the property of the CDC. The following outline will also serve as the QASP ? Quality Assurance Surveillance Plan. ? Deliverable 1: Provide Updated Project Plan for approval by CDC. Within 30 days of effective date of contract ? Deliverable 2: First Interim Progress Report. Within 3 months ? Deliverable 3: Second Interim Progress Report. Within 5 months ? Deliverable 4: Third Interim Progress Report. Within 7 months ? Deliverable 5: Fourth Interim Progress Report. Drafts of all required materials should be provided to the CDC for comment by the time of this report. Within 9 months ? Deliverable 6: Fifth Interim Progress Report ? Final drafts of all required materials should be provided to the CDC by the time of this report for last round of CDC comment. Within 11 months ? Deliverable 7: Final Report and copies of all materials developed for this project. Within 12 months The final report will be due within 12 months of award of contract. All material must be provided electronically, MS Word, Excel, and PowerPoint compatible. The final report shall suitable for submission to CDC clearance and follow CDC guidelines for reports and documents. The final report will include MS Office and PDF versions of all materials, suitable for posting on the internet. The authority for this sole source procurement is FAR part 6.302-1. This is not a request for competitive proposals and no solicitation is available. Firms that believe they can provide the required services are encouraged to identify themselves and give written notification to the Contracting Officer within 15 days from the publication date of this synopsis. A determination by the Government not to compete this proposed action based on responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement.
