- The contractor shall provide FDA approved Positron Emission Tomography (PET)/Computed Tomography (CT) Scan Services to all referred patients of the Department of Veteran Affairs Medical Center, 800 Zorn Avenue, Louisville, KY 40206. The PET/CT scans shall be provided on a scheduled or emergent basis for the DVAMC, Louisville use in determining diagnosis and patient treatment management plans. Services to include all space, equipment, certified personnel, radioactive tracer and incidentals and results/reports. Note: Fusion of separate PET and CT images DOES NOT meet the terms of the contract. Professional Fees are inclusive in the pricing. The scans shall be done at the contractor's facilities within a 15 miles radius of the DVAMC Louisville. The contract must be registered under the NAICS code 621512 Diagnostics Imaging Center. This contract is commercial items procurement for a base year with four one-year options to renew. The following clauses, provisions and any addendum apply: 52.212-1 Instructions to Offerors, 52.212-2 Evaluation, 52.212-3 Representations and Certifications, 52.212-4 Terms and Conditions and 52.212-5 Terms and Conditions to Implement Executive Orders and Statutes.
