Description
The purpose of this amendment is to add the following paragraph to the combined synopsis/solicitation: The evaluation criteria and relative weights to be utilized in order to award this requirement are as follows: delivery (commencement of services),price. Delivery outweighs price as evaluation criteria. However, an integrated assessment will be made for a best-value determination for the government. The contractor?s proposal must include a start date and prices for each series of classes. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the ONLY solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is being conducted under Simplified Acquisition Procedures (SAP) as a Request for Requisition No. F3Q4CA7136AH02. Award will be 100% SB Set-aside; only Small Businesses will be considered. NAICS Code: 621410, business size NTE $9M; Place of Performance/FOB: Robins AFB, GA. DESCRIPTION OF SERVICE: This Performance Work Statement (PWS) describes the work to be performed in the 78th Medical Group, Health and Wellness Center (HAWC), Building 827, Robins AFB, Georgia. This care/service shall provide Child Birth education for Tricare beneficiaries. Civilian Employees on Robins AFB will be considered on a space available basis. The materials provided to each participant will be provided by the contract OB educator. Attendance to classes will be monitored and reported to the HAWC Director. The classes shall follow the following guidelines: 1.1 The educator will conduct a series of classes 1.2 Each series will consist of four classes. 1.3 Each class will be two hours long and will be scheduled in the evening. Each series will cover, at a minimum, the following information: 1.3.1. Discomforts of pregnancy, preterm labor, labor and delivery 1.3.2. C-Sections, epidurals and other medications which may be used during labor and delivery. 1.3.3. Mother and baby care. 1.3.4. The class will be spent touring The Women?s Center at the Houston County Medical Center (Women?s Center) located at 1601 Watson Blvd. 1.3.5. Post-class evaluation will be accomplished and submitted to the HAWC director upon completion of the class. 1.4 The series of classes will be conducted on an ?as needed basis? with at least 2 weeks in between each series of classes. The educator should be a certified childbirth educator or have acceptable experience as an obstetric educator. 2. SCHEDULING CLASSES: The contractor will be available for classes as scheduled. The schedule will be discussed with contractor prior to scheduling classes. Each session will take approximately two hours and will be conducted in the evening. 3. EDUCATION/EXPERIENCE REQUIREMENT: Each educator shall be certified and/or have conducted no less that two complete series within the last 24 months. 4. SECURITY AND PRIVACY REQUIREMENT: The contractor shall use patient information for their designated tasks only. This information shall not be used to create databases or any other product not intended for use specifically for their tasks. All patient information related to their tasks shall be destroyed at the conclusion of the contract. Contactor will adhere to the Privacy Act when providing information. Patient information, no matter how developed, shall be treated as privileged information and shall not be disclosed or revealed in any way for use outside the Medical Treatment Facility without prior written permission by the Chief, Hospital Services. The contractor shall maintain, transmit and retain in strictest confidence, and prevent the unauthorized duplication, use and disclosure of patient information and subcontractors.
