Description
1) This is a sources sought synopsis only. The purpose of this sources sought synopsis is to gain knowledge of potential qualified sources. Responses to this notice shall include the following: (a)company name (b) address (c) point of contact (d) number of employees, as defined by the Federal Acquisition Regulation (FAR) part 19.101 and (e) the amount of annual receipts as defined by the Federal Acquisition Regulation (FAR) part 19. (f) If provider has any Federal Supply schedule that is applicable, then the schedule number, geographic region covered and special item number should be submitted. In addition, the respondent must provide a capability statement that addresses the organizations qualifications ( or individuals) and ability to perform as a prime contractor for the work described below. 2) Description, Personal Services, for One Pediatric Intensive Care Pharmacist. The following is a brief summary of the primary duties that the Pediatric Intensive Care Pharmacist must perform. This list is not all inclusive, Pharmacist shall fill perscriptions, and interpret physician perscriptions as to completeness and appropriatness for Neonatal and Pediatric patients. Intervene with physicians when necessary and document that intervention. Pharmacist review of orders shall include consideration of proper drug (including changes from non-formulary drugs), dose, dosage form, the disease state of patient, and lab results. Pharmacist shall take into consideration all applicable pediatric pharmacokinetics and intravenous compatibility and pediatric dosing parameters. Pharmacist shall participate in Cardiac Arrest Team work. Part of services will be to direct and check admixture preparations, originate and assure completion and approval of Non-formulary Requests when appropriate. Shall oversee bulk compounding, review lab results and perform pharmacokinetics calculations. Conduct TPN education and training, and be a key member of multidisciplinary rounds in intensive care (NICU and PICU) and also wards. Facilities will be at Wilford Hall Medical Center (WHMC), and associated clinics. Start date anticipated is October 2007. 3) Qualifications. The Pharmacist- Health Care Provider (HCP) must be a graduate of an accredited College of Pharmacy, with a Doctorate of Pharmacy degree. Licensure and certification shall be from any US jurisdiction. License or Certifications cannot have been suspended anywhere in the world. Experience shall include at least three years as a pediatric pharmacist during the last four years. Computer software used in facility is Composite Health Care Systems, Pyxis Connect and Pyxis Medstation technology. 4) Place of Performance. These services would be performed at Wilford Hall Medical Center and/or military installations in the San Antonio, Texas area. Anticipate one basic year of service with several option years possible. If a Federal Supply Schedule is applicable, it must include the State of Texas for the authorized performance area.
