Description
Provide single use, sterilized 10 drop IV Sets with 3 Y-type injection sites, with a 4-gang pressure activated valve set on manifold, a pre-pierced Y-type injection site, with roller clamping device, latex free, needle-free, non-vented, luer-lock adapter, approx. 128 inches overall on an on demand, as needed basis, for the VA Medical Center, One Veterans Drive, Minneapolis, MN 55417, in accordance with the Requirements, Schedule of Items, and Specifications of solicitation RFP VA263-07-RP-0107. For comparison the Minneapolis VAMC is currently using Arrow W22331 sets. Current estimated annual usage is 145 cases of 50 each. EVALUATION SAMPLES - Two (2) non-sterile evaluation samples are required and must be received prior to or at the offer due date and time. See IMPORTANT sample requirements in solicitation documents. Any deviation may cause rejection of samples. NIACS: 339113, 500 employees. Solicitation documents available no later than July 30, 2007 with offers due at 4:30 p.m. CT, Thursday, August 30, 2007. Contract effective date estimated to be October 1, 2007 through September 30, 2008, with 4 option years. The Government may award to one or more contractors at its discretion and without discussions. This is an Service-disabled Veteran-owned Small Business set-aside in accordance with PL 109-461. Any SDVOSB business who can respond to this requirement is encouraged to do so. This will be a Best Value selection wherein the Professional Staff of the Minneapolis VA Medical Center will conduct the evaluations of samples. Their decision on which item best meets their needs is final. Written documents must be submitted; electronic or faxed responses will NOT be accepted.
