Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, proposals are being requested and a written solicitation will not be issued. The solicitation number for this procurement is FA4803-07-Q-A265 and is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-17, Defense Federal Acquisition Regulation Supplement Change Notice (DCN) 20070531 and Air Force Acquisition Circular (AFAC) 2007-0531. For informational purposes the North American Industry Classification System code is 333921 Elevator and Moving Stairway Manufacturing. This acquisition is set aside 100% Small Business. This is a firm-fixed price procurement and will be procured using commercial item procedures and award selection will be made based on technical requirements, price and past performance. This Combined Synopsis/Solicitation will be awarded as a Basic Year with Four (4) one year Options. Requirement is for inspection, maintenance and emergency service to seven (7) elevators on Shaw Air Force Base. The elevators shall be serviced twice per month; once during the second week and once during the fourth week of each month. The contractor shall also provide emergency call-back service where the contractor will be required to respond to all emergency calls within two (2) hours, during any hour of the day or night, including weekends and holidays. The contractor shall accomplish all maintenance tasks to meet the requirements of the Statement of Work (SOW). The contractor shall also perform examination of all safety devices and perform safety tests in accordance with the requirements of the American Standard Safety Code for elevators. The contractor shall also keep the elevator machinery clean and furnish special lubricates of the proper type for each application. Elevators are located at Buildings 1990, Bldg 501, Bldg. 1118, Bldg 1122, Bldg 1130, and Bldg 1048 have two elevators. Please see the attached for Statement of Work (SOW). The technical expert will evaluate the quotes on the basis of information furnished by the offeror provided in the RFQ. The following FAR provisions also apply to this acquisition: FAR 52.212-1, Instructions to Offerors Commercial Items; The RFQ should be submitted and shall contain the following information: RFQ number, time specified for receipt of offers, name, address, telephone number of offeror, terms of the expressed warranty, price, any discount terms and acknowledgement of all solicitation amendments (if applicable). Quote shall also contain all other documentation specified herein. Offerors shall include a statement specifying the extent of agreement with all terms, conditions and provisions included in the solicitation. Period of Acceptance for Offerors: The offeror agrees to hold the prices in its offer firm for 90 calendar days from the date specified for receipt of offers, unless another time period is stated in an addendum to the solicitation. Late offers: Request for Quotations or modification of quotes received at the address specified for the receipt of offers after the exact time specified for receipt of offers will not be considered. All offers will be submitted by 1600 (4PM) EST on 20 August 2007. STATEMENT OF WORK (SOW) FOR MAINTAIN ELEVATORS 1. DESCRIPTION OF SERVICES. 1.1 General Information. The contractor shall provide all personnel, equipment, tools, materials, vehicles, supervision, and other items and services to perform inspections and maintenance on the following elevators, at Shaw AFB, SC. Building Manufacturer 1048, Base Hospital, 2 each Southeastern Elevator Co. 1118, 20Th Support Center Southern Elevator Co. 1122, 20th FW Headquarters Southern Elevator Co. 1130, 9th AF Headquarters Southern Elevator Co. 1990, Control Tower Thyssen Elevator Co. 501, Base Education Center Mowrey Elevator Co. 7-stops 1.2 Scope of Work. The elevators shall be serviced twice per month; once during the second week and once during the fourth week of each month. The contractor shall also provide emergency call-back service where by he will be required to respond to all emergency calls within two (2) hours, during any hour of the day or night, including weekends and holidays. The contractor shall accomplish all maintenance tasks to meet the requirements of this SOW. . ? The contractor shall also perform examination of all safety devices and perform safety tests in accordance with the requirements of the American Standard Safety Code for Elevators. ? The contractor shall also keep the elevator machinery clean and furnish special lubricants of the proper type for each application. 1.3 Minor Repairs. The contractor shall provide all minor repairs as a part of this contract at no additional cost to the government. A minor repair is defined as replacing any part when the part, not to include labor, costs no more than $100.00. 1.4 Major Repairs. If repairs cannot be performed under the dollar limitation of minor repairs as described above. Major repairs include furnishing and installing all parts necessary to return any equipment to working order. The cost of major repairs shall be covered in this contract price under Emergency Service/Service Calls line item. ?? The contractor shall furnish the QAP or Contracting Officer a written estimate containing an itemized listing of all major repairs that are required to return the elevator(s) to a safe and operational condition. The written estimate shall contain a cost estimate of all repair work to include a labor and parts breakdown. The major repair work contained in the estimate shall be performed only if approved in writing by the Quality Assurance Personnel (QAP) or Contracting Officer. 1.5 Services Not Covered. The following items of elevator equipment and services are not included in the contract: Refinishing of car enclosure, hoistway doors, frames, sills replacement of car floor covering, buried pipe and buried electrical. 1.6 Emergency Services. The contractor shall be responsible for performing all services requested as emergency services. All emergency services shall be requested and approved by the QAP or contracting officer before the start of work. Contractor shall respond within two (2) hours (nights, weekends or holidays) to any emergency call. Emergency Services: Elevators inoperative. 1.7 Unlimited Call Back Service. Prompt emergency call back service shall be provided within two (2) hours in response to requests by telephone or otherwise from the Contracting Officer or designee, in case of a shutdown or if emergency trouble should develop between regular examinations. This call back service shall be rendered at any hour of any day of the week as requested at no additional charge to the Government. Equipment malfunctions shall be corrected the same day that the service call was issued. If circumstances beyond the contractor?s control preclude resolution of the problem that day, the Contracting Officer or QAP must be notified with an explanation of the delay. Under no circumstances shall the resolution take more than 24 hours after receipt of the call unless an extension is granted by the Contracting Officer or QAP. 2. GENERAL INFORMATION. 2.1 QUALITY ASSURANCE. The government will periodically evaluate the contractor?s performance by appointing a representative(s) to monitor performance to ensure services are received. The government representative will evaluate the contractor?s performance through intermittent on-site inspections and receipt of complaints from base personnel. 2.2 HOURS OF OPERATION All non-emergency work shall be accomplished during the normal duty hours of 0730 through 1630, Monday through Friday, excluding US Government holidays. Shaw AFB recognizes the following holidays: New Year?s Day 1 January Martin Luther King?s Birthday Third Monday in January President?s Day Third Monday in February Memorial Day Last Monday in May Independence Day 4 July Labor Day First Monday in September Columbus Day Second Monday in October Veterans Day 11 November Thanksgiving Day Fourth Thursday in November Christmas Day 25 December NOTE: Any of the above holidays falling on a Saturday will be observed on the proceeding Friday. Holidays falling on a Sunday will be observed on the following Monday. 2.3 SECURITY REQUIREMENTS. 2.3.1 CONTRACTOR PASSES. All contractor employees are required to have a background check performed prior to being issued a base pass which could take up to five (5) days. 2.3.2. KEY CONTROL. The contractor shall establish and implement methods of making sure all keys issued to the contractor by the government are not lost or misplaced and are not used by unauthorized persons. The contractor shall not duplicate any keys and shall immediately notify the QAP or the Contracting Officer any occurrences of lost keys.
