Description
NOTICE OF INTENT TO ISSUE A PURCHASE ORDER. The Centers for Disease Control and Prevention (CDC), the National Institute for Occupational Safety and Health (NIOSH), CDC?s National Center for Environmental Health/Agency for Toxic Substances and Disease Registry (NCEH/ATSDR), CDC?s Office of Strategy and Innovation (OSI), CDC?s coordinating Center for Infectious (CCID), CDC?s Division of Healthcare Quality Promotion (DHQP), the Agency for Healthcare Research and Quality (AHRQ), and the Environmental Protection Agency (EPA) intend to issue a purchase order on a sole source basis to Kaiser Permanente. This is needed for the development and production of report(s) describing existing Kaiser Permanente data fields which capture data on patients, employees and healthcare facilities in relation to green healthcare design. These reports will clarify the existing database variables and identify research opportunities meeting the goals and objectives of participating federal agencies. The databases are the property of Kaiser Permanente therefore no other group can meet the requirements. Responsible sources that believe they possess the expertise and capabilities identified above are encouraged to submit to the Contracting Officer within 15 days from the posting date of this notice their capabilities statement and supporting information in the format they choose, not to exceed 10 pages. Please forward capabilities and supporting information to: Kimberly Groves, Reference, 2007-Q-09897, DHHS/PHS/CDC/NIOSH, 1095 Willowdale Road, Morgantown, WV 26505, or E-mail kpg0@cdc.gov or fax to 304-285-6083. The government will review any/all capabilities statements and supporting information submitted and determine if other qualified sources do exist that could provide the requirement. Information received in response to this notice will be used solely for the purpose of determining whether to conduct a competitive procurement. All vendors must be registered in the Central Contractor Registry (CCR) prior to award of a federal contract. Vendors may register at www.ccr.gov. If no affirmative responses are received within 15 days, in accordance with FAR 13.106-1(b)(1), negotiations will be conducted with Kaiser Permanente as the only source and a purchase order will be issued without any additional notices being posted.
