Description
W91248-07-R-0014 for Respite Care is amended as follows: a. Closing date for submission of proposals remains unchanged. b. Above referenced solicitation package is available at our website: https://doc.campbell.army.mil. Click on Contracting Opportunities; scroll down to Supplies/Services; and then click o Respite Care; solicitation W91248-07-R-0014. c. Offers in response to W91248-07-R-0014 may be submitted electronically to Trudy Colbert, at trudy.colbert@us.army.mil or handcarried to Directorate of Contracting, Building 2172, 13 ½ and Indiana St., Ft. Campbell, KY 42223, at or prior to required c losing time. Offerors are required to submit all information required by the Request for Proposal. d. Full text of referenced clauses may be obtained at www.arnet.gov. e. Following responses are provided to questions received: Q: Statement of Work, Para C.1.1.2.1. through C.1.1.2.6 (and/elsewhere). Are the needed items, equipment, food, etc., provided by the family or the contractor? A: By the family. Q: Statement of Work, Para C.1.1.2.. Does the individual respite care provider need to be limited to only one person, or can more than one person provide respite services? A: More than one provider can care for individual recipients. Q: Is the contractor required to provide transportation of respite care recipients? A: The contractor may provide transportation to the grocery, medical appointments, or other recreational type activities, after an approval by the guardian. If this type of transportation is provided, then contractor must provide appropriate insurance. If the contractor is providing care somewhere other than the recipients home, then the contractor will provide required transportation. Q: Statement of Work, Para C.1.4., Gate Control. Is a gate attendant required? If so, what does the contractor need to know in order to figure usage costs? A: Para C.1.4., delete the last two sentences in their entirety. Para C.1.4. is revised to read as follows: C.1.4. Gate Control. Fort Campbell is a closed post. Unscheduled gate closures may occur at any time. Personnel and vehicles entering or exiting the installation may experience delays. All vehicles and personnel are subject to search. Q: Para C.1.5. Is a CDL required for a provider to transport a recipient in their own vehicle? This item also mentions facility transportation. What license requirement is needed to transport a recipient from for example Hopkinsville to Clarksville? A: Recommend offerors contact the State Department of Motor Vehicles for resolution of this issue. It is the offerors responsibility to determine required licensing requirements. Para C..5. is amended to delete the last sentence in the paragraph. Para C.1.5. is changed to read as follows: C.1.5. Vehicle Registration, Licensing, and Operation. Proper state registration, proof of insurance and a valid drivers license are required to operate a privately owned vehicle on post. Registration with the Provost Marshals Office is required. Deca ls shall be returned to the Provost Marshals Office when: state license is no longer valid; upon sale of the vehicle, or within 3 business days after termination or completion of this contract. Q: Is an individual who is not an employee of the contractor but provides respite care considered a subcontractor and must have Workmens Comp, etc., insurance? A: Offeror should contact the State Workmens Comp office for a determination on workmens compensation insurance. It is the offerors responsibility to resolve this issue. NOTE: THIS NOTICE WAS NOT POSTED TO FEDBIZOPPS ON THE DATE INDICATED IN THE NOTICE ITSELF (02-AUG-2007); HOWEVER, IT DID APPEAR IN THE FEDBIZOPPS FTP FEED ON THIS DATE.
